Mechanical bowel preparation and antimicrobial prophylaxis in elective colorectal surgery in Switzerland—a survey

Abstract: MBP is often used in open and laparoscopic rectal surgery, but not in right colonic resections. Scientific evidence regarding MBP has yielded a rethinking about rigorous bowel preparation regimes. As of now, surgeons in Switzerland are not yet unanimously ready to abandon MBP in elective colorectal surgery. In Switzerland, surgeons are influenced by the benefit of antimicrobial prophylaxis in colorectal surgery.

“…Oral antibiotic bowel preparation is used by only 11%. Other surveys have shown a similar picture in European, Asian and Australasian studies with high use of MBP in rectal surgery and a low rate of oral antibiotic usage [10][11][12][13][14]. This contrasts with patterns of practice in the USA, where surgeons have a high use of both MBP and oral antibiotics [15][16][17].…”

“…Reports of Switzerland and New Zealand are more in line with the recommendations to abandon the practice, with less than half of doctors reporting the use of preparation in colon procedures. However, even in these countries, the use of preparation is common during anorectal surgeries (60-80%) (26)(27) .…”

“…Despite the evidence regarding a lack of benefit from MBP in improved patient outcomes, a high percentage of colorectal surgeons still routinely use MBP [15][16][17]. Their preference may now be supported by a recent study reporting on a 10-year follow-up of a cohort of patients randomized to receive MBP or no MBP while undergoing colonic resection for colorectal cancer.…”

“…Highquality evidence does not support the use of MBP [14]. Despite these data, surveys in the fields of gynecologic and colorectal surgery report a high percentage of surgeons still routinely use bowel preparation [15][16][17][18].…”
