Mechanical ventilation weaning issues can be counted on the fingers of just one hand: part 2

Abstract: Assessing heart and diaphragm function constitutes only one of the steps to consider along the weaning path. In this second part of the review, we will deal with the more systematic evaluation of the pulmonary parenchyma-often implicated in the genesis of respiratory failure. We will also consider the other possible causes of weaning failure that lie beyond the cardio-pulmonary-diaphragmatic system. Finally, we will take a moment to consider the remaining unsolved problems arising from mechanical ventilation a… Show more

“…14 LU allows for the clinical progress of patients admitted to the intensive care unit (ICU) to be monitored (eg, by ultrasound profiling and the LU score) and can predict the probability of successful extubation (the possibility of heart and diaphragm involvement and their ultrasound evaluation are discussed below). 18,38,39 The LU strategy also may allow for the transfer of a patient from the ICU to departments with a lower level of patient care intensity.…”

“…Combining LUS with cardiac and diaphragmatic ultrasound assessment has the potential to improve weaning failure prediction, providing insights into the origin of reduced pulmonary aeration [ 113 , 114 ]. A modified LUS assessment and diaphragmatic thickening assessment were associated with high predictive accuracy of successful extubation, with an AUC of 0.78 and 0.76, respectively, which increased up to 0.83 when considering both assessments together [ 112 ]; however, the relationship between LUS and diaphragm thickening during an SBT seems to vary according to the degree of pulmonary aeration [ 115 ], as well as to the considered subpopulation [ 116 ].…”

“…Intra-abdominal conditions and acute kidney injury (AKI) have shown to worsen outcomes while liberating patients from MV. 22,39 Intra-abdominal hypertension, pneumoperitoneum, hemoperitoneum, abdominal trauma, pancreatitis, end-stage liver disease, liver transplantation can lead to ARDS. 22 As the pressure-volume curve shifts to the right since the compliance decreases from the upward deflection of the diaphragm's initial position; the respiratory workload increases and there is an increase in inspiratory pressures required to ventilate.…”
