Self-treatment for eye diseases is very common in most developing countries yet there has been little investigation of such attitudes and practices. In many settings, people do not proceed beyond self-treatment and do not receive care from either traditional healers or Western eye care providers. Visual impairment and blindness can be the result. We conducted population-based survey of use of eye care services and self-treatment in two districts of Malawi. Adults were administered a detailed interview regarding their use of eye care services (Western and traditional as well as self-treatment) and their knowledge and use of traditional eye medicines. Self-treatment was defined as use of either Western or traditional medicines by the individual for their most recent eye condition. Only eye conditions that were considered severe by the study subjects were correlated with treatment options. Interviews were carried out among 800 adults in the study areas. Self-treatment was reported for the last episode of eye disease by 39.8% of the study population. Factors associated with self-treatment included sex, religion and socioeconomic status. Even though 76.8% of the respondents reported treatment from the health center or hospital to be the least expensive option, many opted for self-treatment first. Among those opting for self-treatment 72% used traditional eye medicines. Even among cases that individuals considered to be quite severe (these included cataract, trachoma and conjunctivitis), self-treatment was the option of choice in 22.2% of cases.

Papers from the session, combined with additional contributions, were collected in a special issue of Wetlands Ecology and Management. The current paper reviews ecological and hydrological characteristics of papyrus wetlands, summarizes their ecosystem services and sustainable use, provides an overview of papyrus research to date, and looks at policy development for papyrus wetlands. Based on this review, the paper provides a synthesis of research and policy priorities for papyrus wetlands and introduces the contributions in the special issue. Main conclusions are that (1) there is a need for better estimates of the area covered by papyrus wetlands. Limited evidence suggests that the loss of papyrus wetlands is rapid in some areas; (2) there is a need for a better understanding and modelling of the regulating services of papyrus wetlands to support trade-off analysis and improve economic valuation; (3) research on papyrus wetlands should include assessment of all ecosystem services (provisioning, regulating, habitat, cultural) so that trade-offs can be determined as the basis for sustainable management strategies ('wise use'); (4) more research on the governance, institutional and socio-economic aspects of papyrus wet-lands is needed to assist African governments in dealing with the challenges of conserving wetlands in the face of growing food security needs and climate change. The papers in the special issue address a number of these issues.

Pirandai uppu (PU) a herbo mineral formulation has been employed as a traditional siddha remedy for diarrhea since long time. Diarrheal diseases remain one of a leading cause of morbidity and mortality in the world, particularly in developing countries. As a mandate, steps were taken to evaluate safety profile of PU in animal model under OECD guidelines. Acute toxicity studies done on female wistar albino rat under OECD guidelines 423 and 28 days repeated oral toxicity studies done on both sex of wistar albino rat under OECD guidelines 407. Acute oral toxicity study of PU revealed no mortality at the dosage of 2000 mg/kg body weight and the median lethal dosage of PU is estimated above 2000 mg/kg body weight. Repeated oral toxicity study of PU does not exhibit mortality at the high dosage of 200 mg/kg body weight given up to the period of 56 days including 28 days of drug administered. At the end of 28 days no specific changes are observed in hematological, hepatic, renal and other biochemical parameters. No gross morphological and histological changes are observed in the organs. The above studies recommend that PU is a safest drug under intended human adult dosage (500 mg) as illustrated in the literature.
