Mesenteric panniculitis associated with abdominal tuberculous lymphadenitis: a case report and review of the literature

Abstract: Mesenteric panniculitis is a rare disease characterized by chronic non-specific inflammation of the mesenteric adipose tissue. The specific aetiology of the disease is previously unknown. A case diagnosed as mesenteric panniculitis is presented. The cause was biopsy-proved abdominal tuberculous lymphadenitis. To our knowledge, mesenteric panniculitis associated with tuberculosis infection has not been reported previously in the literature. Thus, we would like to present the first case and describe CT features … Show more

“…Several atypical microorganisms such as Cryptococcus neoformans and Mycobacterium genavense have been reported in HIV patients in relation to MP [14,15]. On the other hand, M. tuberculosis and Vibrio cholera are suspected to be the infectious agents that cause MP in immunocompetent patients [16,17]. In addition, various acute abdominal inflammatory disorders in which enteric bacteria was involved in the etiopathogenesis, such as diverticulitis and acute appendicitis, have been reported in relation to MP [18].…”

“…Neutrophilia, increased erythrocyte sedimentation rate or anemia have been reported occasionally in the retractile mesenteritis stage [6] . Some reports even go as far as stating that few or none of the patients with mesenteric panniculitis can be diagnosed correctly before operation [4,14] . However, with the advent of imaging technology like high-resolution CT or magnetic resonance imaging, distinguishing mesenteric panniculitis from other mesenteric diseases with similar imaging features such as carcinomatosis, carcinoid tumor, lymphoma, desmoid tumor, and mesenteric edema seems possible and feasible [15,16] .…”

“…Although the aetiology remains obscure, association with other diseases has been described. AIDS [14] , non-Hodgkin lymphoma [15] , tuberculitis [1] , cholesterol ester storage disease [16] have been found together with sclerosing mesenteritis. Drug therapy is not standardized, and should be based on the stage of the disease.…”
