Mesothelial Splenic Cyst.

Abstract: A 29-year-old female was admitted because of thrombocytopenia and an abdominal mass which was palpated 5 finger breadths below the left costal margin. A computed tomographic scan of the abdomen revealed splenomegaly with a large calcified cystic lesion. The spleen was removed and the pathological examination disclosed that the cyst was monolocular and the wall was of mesothelial origin. (Internal Medicine 32: 672-674, 1993)

“…Most benign, nonparasitic cysts of the spleen are endothelial or epidermoid, while mesothelial cysts are very uncommon. 11 In a study of 651 cases of nonparasitic splenic cysts, only 4 cases (4.9%) were mesothelial. 12 The differential diagnosis of primary malignant mesothelioma of the spleen includes some biologically distinct entities, namely, localized mesothelioma, secondary involvement of the spleen by a peritoneal or pleural mesothelioma, primary splenic tumors and metastases.…”

“…Clinically, primary cysts occur predominantly in children and young adolescents; often asymptomatic until they assume large sizes, they may then present with local or referred pain, abdominal distension compression of adjacent structures and rarely as thrombocytopenia 1 6 7…”

“…Capsular surface mesothelial invagination with subsequent cyst formation,6 embryonic inclusion of epithelial cells from adjacent structures, epithelial cell metaplasia from adjacent structures or vascular endothelium from peritoneal inclusions are some of the theories put forth to explain the genesis of these congenital cysts 1…”
