Metabolic abnormalities in an early psychosis service: a retrospective, naturalistic cross-sectional study

Abstract: Over a third of young patients being treated for their first episode of psychosis either had metabolic syndrome or showed metabolic abnormalities. Treatment duration related to higher BMI and greater prevalence of metabolic syndrome. Detection of metabolic complications after treatment instigation in patients with first-episode psychosis will permit early intervention with lifestyle or drug interventions in those at risk of significant physical health morbidity.

“…Hypotheses on the association between schizophrenia and diabetes (adapted from Leucht et al [2007aLeucht et al [ , 2007b of the study group varied, however male patients appeared to be over represented. In most studies the mean age of participants in the study group was over 40 years of age, and only a few studies referred to patients with first episode schizophrenia [Attux et al 2007;Saddichha et al 2007;De Hert et al 2008b;Saddichha et al 2008;Curtis et al 2011]. The predominant diagnosis of patients studied was schizophrenia, however a great number of studies also included patients with schizoaffective disorder and other psychotic disorders.…”

“…13 Of note, both diabetes and dyslipidaemia can occasionally appear even in the absence of weight gain, underlining the importance clinically of being alert to the possibility of serious metabolic disturbance occurring in those on antipsychotic medication who have not gained weight. 14 Weight gain and obesity are of particular concern because of their potential to adversely affect cardiometabolic risks.…”

“…Intervencije su moguće i dokazano efikasne na tri nivoa: 1. intervencije u životni stil (prehrana, tjelesna vježba, prestanak pušenja i dr.), 2. uvođenje specifičnih lijekova za arterijski pritisak, šećer, lipide i dr. i 3. racionalni izbor psihofarmaka [3,7,26,27]. Međutim, i pored brojnih dosadašnjih istraživanja i dalje ostaje otvoreno pitanje: ''Da li psihički poremećaj dovodi do metaboličkog sindroma ili biohemijske promjene kod metaboličkog sindroma dovode do psihičkih promjena, ili je metabolički sindrom samo nuspojava psihofarmakoterapije nezavisno od patogeneze ovih oboljenja?…”
