Metastasis to the gallbladder: A single-center experience of 20 cases in South Korea

Abstract: AIM:To evaluate the clinicopathologic characteristics of patients with metastases to the gallbladder (MGBs). METHODS:We performed a single-center retrospective study of 20 patients with MGBs diagnosed pathologically from 1999 to 2007. RESULTS:Among 417 gallbladder (GB) malignancies, 20 (4.8%) were MGBs. The primary malignancies originated from the stomach (n = 8), colorectum (n = 3), liver (n = 2), kidney (n = 2), skin (n = 2), extrahepatic bile duct (n = 1), uterine cervix (n = 1), and appendix (n = 1). “…In the western medical literature, the most common primary tumor to metastasize to the gallbladder is melanoma, which represents 60% of all gallbladder metastases (Fig 10) (57). In the Asian medical literature, the most common source of gallbladder metastases is gastric cancer (58). Cancers of the kidney and lung may hematogenously spread to the gallbladder, whereas hepatocellular carcinoma and cholangiocarcinoma may directly invade the gallbladder (Fig 11).…”

“…The patients with isolated GBM from the other malignancies generally present with obstructive jaundice if the tumor is located in the common bile duct or with upper abdominal pain mimicking symptomatic cholecystolithiasis (3,11,12), but asymptomatic cases were also reported (4,12). Our patient presented with right upper abdominal pain and fever without obstructive jaundice; therefore, our patient was compatible with the literature.…”

“…Cholecystectomy with R0 resection has been demonstrated to be the only factor that increases survival, mainly in isolated cases of metastasis (4,10). Acute cholecystitis as a clinical presentation is associated with poor prognosis (10).…”
