Methods for the Classification of Leprosy for Treatment Purposes

Abstract: The World Health Organization advocates 2 leprosy treatment regimens on the basis of disease classification (as multibacillary or paucibacillary) by skin lesion count. This method, which, in the Philippines, results in a high prevalence (78%) of patients with multibacillary leprosy, was directly compared with classification using standard histopathological and microbiological criteria in 264 currently untreated patients with leprosy. Of those whose leprosy was classified as paucibacillary, 38%-51% of patients … Show more

“…Tests could greatly aid general practitioners in their evaluation of suspected cases. This would appear particularly prudent in regions where a large proportion of patients present as MB leprosy cases, such as the Philippines (31). Importantly, the ability to objectively read and quantify NDO-LID test results using a Smart Reader eliminates the need for prior training/experience in interpreting test results.…”

“…Leprosy is a contagious, granulomatous and chronic infectious disease caused by Mycobacterium leprae, an obligate intracellular bacillus that affects mainly the skin and peripheral nerves [1][2][3] . It can be better understood when regarded as an association of two diseases: first, a chronic infection that induces a cell-mediated immune response; second, a peripheral neuropathy which is triggered by infection and followed by a series of immunological events, whose evolution, sequels and disabilities often last many years, even after infection has ceased [4][5][6][7] .…”

“…Tuberculoid leprosy (TT) was defined as a paucibacillary (PB) patient, it means BI=0, with 1 to 5 hypo pigmented or erythematous sharply demarcated macules, with hypoesthesia, and involvement of one peripheral nerve identified by enlargement and/or localized pain (24,25). Lepromatous leprosy (LL) was defined as a multibacillary (MB) patient, IB>0, with more than 5 skin lesions, nodules, plaques or papules, loss of sensitivity and compromised of several nerves (24,25).…”
