Methotrexate as a steroid sparing agent for asthma in adults

Abstract: Cochrane Database of Systematic Reviews Authors' conclusions Methotrexate may have a small steroid sparing e ect in adults with asthma who are dependent on oral corticosteroids. However, the overall reduction in daily steroid use is probably not large enough to reduce steroid-induced adverse e ects. This small potential to reduce the impact of steroid side-e ects is probably insu icient to o set the adverse e ects of methotrexate.

“…IgG, were studied as corticosteroid-sparing agents with the aim of reducing the dose of maintenance OCS. Although these agents may be considered to improve corticosteroid insensitivity, their efficacy is uncertain, and they are associated with significant side-effects (see GRADE question below) [179][180][181][182][183].…”

“…Two metaanalyses of the steroid-sparing effect of low-dose methotrexate showed a small overall benefit but a relatively high frequency of adverse effects [128,129]. This small potential to reduce the impact of glucocorticosteroid side-effects is probably insufficient to offset the adverse effects of methotrexate [130]. Cyclosporin [131] and gold [132,133] have also been shown to be effective in some patients.…”

“…Therefore, tapering oral glucocorticosteroids to the lowest effective dose is of high priority [69]. Classical steroid sparing treatments including oral gold [71], methotrexate [72] and ciclosporin [73] have not gained acceptance because of limited effects and significant side-effects.…”
