Minimally Invasive Complete Mesocolic Excision with Extended Lymphadenectomy for Colon Cancer

Abstract: Concepts• Complete mesocolic excision and central vascular ligation (CME + CVL) refers to en bloc dissection of the colon and mesocolon along the embryologic planes with preservation of the mesocolic envelope, central ligation of the feeding vessels, extended lymphadenectomy, and adequate bowel resection. • CME + CVL most closely matches Japanese D3 lymphadenectomy which requires en bloc resection of paracolic (D1), intermediate (D2), and central (D3) nodal stations along the feeding vessels. • CME + CVL/D3 di… Show more

“…For sigmoid or distal descending colon tumours, mobilization of the splenic flexure, descending and sigmoid colon is performed according to established principles for a conventional resection with D2 lymphadenectomy (medial to lateral or lateral to medial). For a CME with CVL/D3 dissection, 30 the IMA is dissected at its origin from the aorta and skeletonised of all fibrofatty lymphatic tissue, while preserving the superior hypogastric plexus. The IMA is divided at its origin, the IMV is divided at the lower border of the pancreas and the left mesocolon between the two ligated vessels is released from the retroperitoneum.…”
