Mixed Th1 and Th2 Mycobacterium tuberculosis-specific CD4 T cell responses in patients with active pulmonary tuberculosis from Tanzania

Abstract: Mycobacterium tuberculosis (Mtb) and helminth infections elicit antagonistic immune effector functions and are co-endemic in several regions of the world. We therefore hypothesized that helminth infection may influence Mtb-specific T-cell immune responses. We evaluated the cytokine profile of Mtb-specific T cells in 72 individuals with pulmonary TB disease recruited from two Sub-Saharan regions with high and moderate helminth burden i.e. 55 from Tanzania (TZ) and 17 from South Africa (SA), respectively. We sho… Show more

“…Furthermore, in both SM + and SM − individuals in the LTBI group, proliferating Mtb-specific CD4 T cells expressed both TH1 and TH2 markers and expressed TH1 cytokines upon restimulation with PMA and ionomycin. This is consistent with previous studies which have reported of co-expression of lineagespecific transcription factors and transcriptional profiles in Mtbspecific CD4 T cells (23,60,62). Due to the low frequency of Mtbspecific proliferating CD4 T cells in TB group, consistent with previous reports (52), we were not able to evaluate TH1 and TH2 profiles of proliferating Mtb-specific CD4 T cells in individuals with active TB.…”

“…Interestingly, SM + active TB individuals did have more GATA3 + CCR4 + TH1 cytokine + Mtbspecific cells than SM − active TB individuals. This is consistent with a recent study in Tanzania which reported mixed TH1/TH2 phenotypes in Mtb-specific CD4 T cells in individuals with active TB and helminth coinfection, although the participants in this study were not stratified by helminth species (23). It is difficult to determine whether this is due to TH1 cytokine + cells being skewed to a TH2 phenotype or whether phenotypically TH2 cells are being reprogrammed to produce TH1 cytokines.…”

“…According to the latest WHO Global TB Report, Mtb has led to 10 million cases of TB in 2017, and is also one of the top 10 causes of mortality with ∼1.6 million (1.3 and 0.3 million in HIV-negative and HIV-positive individuals, respectively) deaths in 2017, which translates to ∼4000 deaths each day (Tiberi et al, 2018;World Health Organization [WHO], 2018). Although universal Bacillus Calmette-Guérin (BCG) is administered in many countries, the vaccine is only effective against disseminated TB in children, and it's efficacy in adults largely remains controversial (Colditz et al, 1994;Zwerling et al, 2011;Amelio et al, 2017).…”
