Aim:The aim of study was radiographic assessment of the effect of conservative access cavity preparation and different root canal tapers preparation on apical transportation of curved root canals.Materials and methods: we used 45-extracted mandibular permanent molar having mesial root canal curvature range .CBCT done to determine the root curvature and anatomy for access cavity standardization. Teeth were selected and divided into three groups, according to access cavity preparation design into: Group1 (Traditional access cavity), Group2 (Conservative access cavity), Group3 (Truss access cavity). Pre instrumentation standardized periapical x-ray were done with #15 k-file inserted inside mesiobuccal canal Then teeth were divided according to rotary file system used for canal instrumentation into three subgroups ( Oneshape , ProTaperNext and Trunatomy). After access preparation done all teeth were instrumented using OneShape 25/0.6 , Protaper Next X1,X2 and Trunatomy small (#20) and Prime (#26) . Post instrumentation standardized radiograph were taken with (OneShape 25/0.6, ProTaper Next X2 and Trunatomy Prime (#26)) file inserted inside mesiobuccal canal. Superimposition of pre and post instrumentation radiographs were done using Adobe Photoshop program . Image-J software was used to measure angle of deviation between the 2 files. Statistical analysis for image analysis results was performed by applying ANOVA test (one way) followed by Tukey post hoc test.Results: Conservative access cavity showed highest angle of deviation when used with OneShape and Trunatomy while showed the lowest angle of deviation with ProTaper next .Regarding the type of file used, OneShape showed the highest angle of deviation with conservative and truss access cavities and lowest with traditional access cavity while ProTaper next showed the lowest angle of deviation with conservative and Truss access cavities .

Introduction: Biomechanical preparation has gotten easier over time with the development of nickel-titanium (NiTi) rotary instruments. Despite their benefits, research has shown that these files frequently result in microcracks in the root canal dentin, which can fracture the roots. Such mishaps should be prevented, as they compromise the integrity of the root and reduce the long-term survival of endodontically treated teeth. Materials and methods: This study was conducted at Government Dental College and Hospital, Patiala, Punjab, India. Eighty permanent mandibular premolar teeth were included. All the roots were inspected for any pre-existing cracks or craze lines under a stereomicroscope. The teeth were decoronated and then divided into four groups (n = 20): Group I: TruNatomy, Group II: Neoendo Flex, Group III: ProTaper Gold, and Group IV: 2Shape. The samples were instrumented according to the group to which they belonged. The roots were then sectioned horizontally at 3 mm and 6 mm from the apex and examined under a stereomicroscope at 40x for the presence of microcracks. Results: The data were analyzed using the IBM SPSS Statistics for Windows, version 26 (released 2019; IBM Corp., Armonk, New York, United States). A chi-square test was applied, and the level of significance was set at p < 0.05. The highest incidence of microcracks was associated with ProTaper Gold (65%), followed by Neoendo Flex (45%), TruNatomy (20%), and 2Shape (20%). Conclusion: All rotary instruments resulted in dentinal damage. ProTaper Gold exhibited the highest frequency of dentin cracks. TruNatomy and 2Shape exhibited satisfactory results with minimal crack formation.

The study aimed to find the incidence and awareness of endodontic instrument separation and its management among dental house officers, postgraduate trainees, demonstrators, consultants, and general dentists. Methods: This online questionnaire-based cross-sectional study was conducted with the approval of the IRB in private and public dental hospitals and dental clinics in Punjab. The authors developed the survey tool, which comprises 24 closed-ended items regarding demographics, the incidence of file separation, and awareness about its management. The data were analyzed using IBM SPSS version 24. The Chi-Square Test was used to compare percentages of categorical variables. Results: Postgraduate trainees experienced the most instrument separations (43.6%), made the most retrieval attempts (49.2%), and experienced the most secondary errors during retrieval (52.1%) (p<0.001). Around four out of ten respondents always informed the patients (39.6%) and department (41.6%) about errors. Manual files (69.8%), stainless steel files (75.8%), and short files (60.4%) were more frequently separated, and the most frequent cause was older fatigue files (57.7%). Manual files were more frequently broken in public dental institutes (p=0.003). Two-thirds of the file separations (72.5%) occurred during cleaning and shaping in the apical third of molars (65.1%), especially in mesiolingual canal (56.4%). Bypass attempt was the most common in symptomatic teeth (47.7%). Conclusions: Preventive approaches such as limiting file reuse and constructing a glide path can reduce the occurrence of file separation. Operators should be familiar with the number of uses of the instrument before fatigue and should be trained through workshops and refresher courses.
