Modified In Situ Pericardial Rerouting Technique for Scimitar Syndrome Repair

Abstract: Scimitar syndrome repair represents a challenge due to the high incidence of postoperative pulmonary venous obstruction associated with classic surgical strategies. In situ pericardial rerouting technique has been considered a promising alternative approach due to its simplicity and excellent midterm results. Access to the left atrium can be difficult in young patients with severe dextrocardia and hypoplastic right lung. We describe a modification of the original rerouting technique in which the atrial septum … Show more

“…90 Repair when the pulmonary veins are distant from the left atrium is challenging, and may eventuate in a higher prevalence of pulmonary or inferior caval venous stenosis, or intracardiac baffle obstruction. [3][4][5]13,58,61,64,89,90 A myriad of surgical techniques has been described for managing distant anomalous pulmonary veins, including mobilization and direct CHOWDHURY ET AL.…”

“…Thus, direct implantation is more reasonable for horizontal scimitar veins. 61 Opinion is divided regarding the approach to aortopulmonary collateral arteries. 24,76 While some have demonstrated the beneficial effects of coil embolization on reduction of pulmonary hypertension and congestive heart failure, others have an inconsistent experience.…”

“…No patients in the series developed diaphragm paralysis or phrenic nerve palsy. Outcomes after other methods for APV drainage distant from the left atrium are infrequently reported 3,4,[8][9][10][11][12] and variable. Possible surgical techniques employed for managing distant anomalous pulmonary veins include mobilization and direct reimplantation into left atrial wall, use of interposition grafts, and creation of an intra-atrial baffle from the orifice of the anomalous veins to the left atrium through a preexisting or constructed atrial septal defect.…”

“…Scimitar syndrome is a congenital heart disease involving the pulmonary venous connection to the right heart in association with pulmonary abnormalities. Symptoms include hypoplasia of the right lung and pulmonary branch along with the sequestration of the right lower lobe as well as the large aortic collaterals (4,5). The infantile form usually requires surgical intervention; however, the adult form is usually asymptomatic and surgical repair is dependent on the severity of the left-right shunt (4,6,7).…”
