Modified Scleral Suture Fixation Technique for Foldable IOL with Fenestrations in the Proximal Part of the Haptics

Abstract: It is possible to decrease the risk of tilt of the IOL with fenestrations in the proximal part of the haptics by modifying the technique so that the 2 needle passes through the sclera for each haptic are done in the same meridian, resulting in a layout of the external part of the suture being radial to the limbus.

“…[ 21 ] Some SPA IOLs have closed haptics or eyelets and spaces that facilitate suturing and make the surgery safer. [ 3 16 21 25 26 27 ] Using a cow-hitch knot with those kinds of IOLs may be less likely to irritate the iris.…”

“…[ 35 ] Many studies have described scleral fixation of different SPA acrylic IOLs with different techniques. [ 3 4 5 16 21 25 26 27 28 ] None of these eyes showed any complications, such as chronic low-grade inflammation, iris chafing, pigment dispersion, UGH syndrome, IOL decentration, or IOL tilt. Only CME was observed in three eyes,[ 3 16 ] and one of them[ 16 ] was resolved with intravitreal triamcinolone acetonide injections.…”

“…This avoids slippage of the suture and easy placement of the needle. Other techniques described in literature for sclera fixation of IOLs are the Lewis technique [12], Kaluzny modification of scleral fixation of IOL [13] and the technique introduced by Seki et al [14].…”
