Molecular sensitization patterns and influence of molecular diagnosis in immunotherapy prescription in children sensitized to both grass and olive pollen

Abstract: Double sensitization to grass and olive pollen is common in Spain and also occurs in the pediatric population. Molecular diagnosis using specific IgE may help improve immunotherapy selection in polysensitized patients.

“…In a prospective study with 476 patients with polysensitization to inhaled and food allergens, 175 the molecular diagnostic approach changed the prescription of AIT in about 50% of patients, which also decreased the cost of AIT. Martinez-Canavate Burgos et al observed in 281 children with double sensitization to olive and grass, 300 and Del-Rio Camacho et al 301 observed in 70 children with various sensitizations, that the AIT prescription was changed in more than 50% after PAMD@; Del-Rio Camacho et al also noted that the proportion of AIT prescriptions for a single allergen increased from 18% to 51%. Similarly, the use of PAMD@ changed prescriptions in 42%-48% of 651 children in an Italian study.…”

“…The ability of defining the genuine sensitization profile (thus to rule out the confounding role of pan‐allergens as cause of false positivity at standard tests) also allowed to obtain a more and more detailed prescription profile. It was clearly documented that the molecular‐based diagnosis could lead to a significant change, estimated in about 40%‐50% in the SLIT prescription as compared to the standard diagnostic approaches . Notably, it was shown that as larger is the sensitization profile, as greater is the AIT failure .…”

“…Grass and olive pollen seasons in Spain overlap and so it is difficult to identify the true nature of the allergen in patients with seasonal AR. The molecular sensitization pattern revealed that most patients (76.2%) in a Spanish paediatric population were sensitized to both major grass (PhI p 1 + 5) and olive (Ole e 1) pollen allergens …”

“…The molecular sensitization pattern revealed that most patients (76.2%) in a Spanish paediatric population were sensitized to both major grass (PhI p 1 + 5) and olive (Ole e 1) pollen allergens. 120 Very preterm-born children are predisposed to respiratory diseases due to improper lung maturation. Protein supplementation of preterm formula and maternal milk improves lung function at 6 years of age.…”
