Monitoring hepatic cholesterol 7α-hydroxylase activity by assay of the stable bile acid intermediate 7α-hydroxy-4-cholesten-3-one in peripheral blood

Abstract: We describe an accurate method for monitoring the enzymatic activity of hepatic cholesterol 7 ␣ -hydroxylase (C7 ␣ OH; CYP7A1), the rate-limiting and major regulatory enzyme in the synthesis of bile acids. Assay of 7 ␣ -hydroxy-4-cholesten-3-one (C4), an intermediate in bile acid synthesis, revealed that the level of C4 in peripheral blood serum or plasma showed a strong correlation to the enzymatic activity of hepatic C7 ␣ OH, both at steady-state conditions ( r ‫؍‬ 0.929) as well as during the rapid changes … Show more

“…The two clinically desired results were a lowering of serum LDL cholesterol levels, and based on measurements of established bio-markers for bile-acid and cholesterol synthesis (18,19), an increase in bile acid synthesis in the absence of an effect on cholesterol synthesis. In the 2-week study, KB2115 lowered serum cholesterol levels to a maximal level of 40% reduction.…”

“…Serum lipids including cholesterol (total, LDL, HDL), triglycerides, free fatty acids, apoA-I, apoB, and lipoprotein a were measured in fasting samples. Also changes in bile acid and cholesterol synthesis rates were assessed by measurements of C4/ cholesterol and lathosterol/cholesterol as described (18,19).…”

“…Thus, elobixibat decreases the plasma LDL‐C level. Concurrently, elobixibat increases the level of the intermediate product of BA synthesis: plasma 7α‐hydroxy‐4‐cholesten‐3‐one (C4), which is a surrogate of the hepatic BA synthesis rate 14, 15. Another factor likely to increase BA synthesis is food intake, which may influence the pharmacological action of elobixibat.…”
