Neurodevelopmental impairments amongst the extremely low birth weight (ELBW) or extremely preterm population are well recognised. However, support services for these children who have mild impairments are lacking. This research aimed to explore the performance of a group of non-disabled ELBW or extremely preterm children with minimal/mild impairments over a 12 month period at a critical time-point, coinciding with the commencement of formal education. Performance of these children was established, as well as whether impairments persist. Persistence of impairments may suggest a widening gap between performance of these children and expected performance of term born peers. Additionally, this research aimed to investigate the short term and longer term impact of group-based physiotherapy intervention compared to standard care. SubjectsFifty children born ELBW and/or extremely preterm (26 males; mean birth weight: 838.8g, SD 174.8g; mean gestational age: 27.0 weeks, SD 2.0 weeks) with minimal/mild motor impairments and a mean age (corrected for prematurity) of 50 months (SD 2.6 months) at baseline. Twenty-four children were randomised to intervention and 26 children to standard care. MethodsChildren were assessed by paediatric physiotherapists blinded to group allocation. A set testing protocol was used. Baseline assessment included: Movement Assessment Battery for Children second-edition (MABC-2), tests of postural stability and limb strength, Child Behavior Checklist (CBCL) and Beery Visual-Motor Integration Test 5 th Edition (Beery VMI). Following baseline assessment, children were randomly allocated to intervention or standard care. All children were assessed on motor and postural tests at the conclusion of the intervention. Goal Attainment Scaling (GAS) was utilised within the intervention group. At one year post baseline assessment, all children were assessed on baseline tests, as well as Peabody Picture Vocabulary Test 4 th Edition (PPVT-4). The group had low average motor co-ordination, lower than expected postural stability and strength, and acceptable behaviour. Mean percentile rank on MABC-2 was 31%.However, 30% of children had a score less than or equal to the 15 th percentile, indicating at risk of or having a definite motor problem and these children had poorer postural outcomes compared to children with normal motor performance. Behaviour was within normal range according to performance on CBCL, although between 11-15% of children had a score within the clinical range. Behaviour of males was poorer than that of females. Study IV: GAS to explore effect of group-based physiotherapy intervention on personal growth of non-disabled ELBW children Intervention was effective with GAS mean T score of the group exceeding the expected level (mean=58.17, SD=0.82). Goal attainment was associated with motor co-ordination 4 and children with better motor skills post intervention, were more successful in reaching goals. Females improved more than males on GAS. Developmental Coordination Disorder and Its Association With Developmental Comorbidities at 6.5 Years in Apparently Healthy Children Born Extremely Preterm

IMPORTANCE There are concerns that apparently healthy extremely preterm children face a risk of developing motor impairments, such as developmental coordination disorder. OBJECTIVE To evaluate the prevalence of developmental coordination disorder and associated comorbidities in a national cohort of apparently healthy children born at 22 to 26 gestational weeks, compared alongside term-born peers. DESIGN, SETTING, AND PARTICIPANTS This prospective, population-based cohort study included all children who were consecutively born at 22 to 26 gestational weeks in Sweden from April 1, 2004, through March 31, 2007. At 6.5 years, 441 preterm children were evaluated alongside 371 controls. A total of 275 preterm children (62.4%) and 359 term-born children (96.8%) did not have neurodevelopmental disabilities. Motor assessments were completed for 229 of 275 preterm children (83.3%) and 344 of 359 (95.8%) term-born children, who composed the final study sample. MAIN OUTCOMES AND MEASURES Developmental coordination disorder was defined as a score of the fifth percentile or lower on the Movement Assessment Battery for Children-Second Edition scale, using control group scores. Assessment tools included the Wechsler Intelligence Scale for Children-Fourth Edition, the Brown Attention-Deficit Disorder Scales, the Five to Fifteen questionnaire, and the Strengths and Difficulties questionnaire. RESULTS Of the 229 extremely preterm children and 344 term-born controls who underwent motor assessments, 115 (50.2%) and 194 (56.4%) were boys, respectively. Developmental coordination disorder was present in 85 of 229 (37.1%) preterm children and in 19 of 344 controls (5.5%) (adjusted odds ratio [OR], 7.92; 99% CI, 3.69-17.20). When preterm children with developmental coordination disorder were compared with term-born peers, the risk was increased for total behavioral problems, internalizing, externalizing, attentional problems, hyperactivity, perceptual problems, executive dysfunction, and poor social skills, with adjusted ORs varying from 2.66 (99% CI, 1.09-6.48) for time concepts to 9.06 (99% CI, 3.60-22.8) for attentional problems (all P < .01). When preterm children with and without developmental coordination disorder were compared, preterm children with developmental coordination disorder had more behavioral problems; the adjusted OR for total behavioral problems was 2.71 (99% CI, 1.15-6.37); for externalizing problems, 2.80 (99% CI, 1.10-7.12); for inattention, 3.38 (99% CI, 1.39-8.18); and for combined attention/hyperactivity problems, 3.68 (99% CI, 1.47-9.16) (all P < .01). Parents underestimated the children's motor problems and only a few of the children had received psychological care or physiotherapy. CONCLUSIONS AND RELEVANCE Children who were born extremely preterm faced a high risk for developmental coordination disorder with associated comorbidities. Our findings support the importance of a structured follow-up of motor function, behavior, and cognition.

Preterm birth is characterized by multiple interacting atypical constraints affecting different aspects of neuropsychological development. In the first years of life, perceptual, motor, and communicative-linguistic abilities, as well as attention, processing speed, and memory are affected by preterm birth resulting in cascading effects on later development. From school age to adolescence, a catch-up of simpler competencies (i.e., receptive lexicon) along with a more selective effect on more complex competencies (i.e., complex linguistic functions, math, motor, and executive functions) are observed, as well as a relevant incidence of behavioral outcomes. A wide heterogeneity in preterm children's neuropsychological profiles is described depending on the interaction among the degree of neonatal immaturity, medical complications, neurological damages/alterations, environmental and social factors. Severe neuromotor and sensory damages are not frequent, while low severity impairments are common among preterm children. It is argued that developmental pathways of preterm children are atypical, and not merely delayed, and are characterized by different developmental patterns and relationships among competencies.
