MR Contrast Agents for Liver Imaging: What, When, How

Abstract: The major classes of contrast agents currently used for magnetic resonance (MR) imaging of the liver include extracellular agents (eg, low-molecular-weight gadolinium chelates), reticuloendothelial agents (eg, ferumoxides), hepatobiliary agents (eg, mangafodipir), blood pool agents, and combined agents. Mechanisms of action, dosage, elimination, toxic effects, indications for use, and MR imaging technical considerations vary according to class. Gadolinium chelates are the most widely used. Ferumoxides are a us… Show more

“…The introduction of breath-hold sequences with gadolinium chelates have facilitated the development of dynamic contrast-enhanced imaging of the liver in clinical practice. 27,28 In turn, a number of studies have described changes in hepatic parenchyma texture after contrast-enhanced imaging by MRI (Table 1). [27][28][29][30][31][32][33][34][35] Early patchy enhancement may correlate with increased numbers of macrophages and necroinflammation on liver histology.…”

“…These properties may allow for improved tumor imaging. 21 Intravenous contrast-enhanced abdominal imaging is an important screening and diagnostic modality in liver transplant candidates. Given the potential risk for CIN associated with iodinated CT contrast and the effect of renal insufficiency on mortality in patients with cirrhosis, alternate contrast modalities for abdominal imaging need to be examined for this patient population.…”
