MRI in patients with axillary metastases of occult breast carcinoma

Abstract: In 4 women with adenocarcinoma metastasis in an axillary lymph node and no primary tumor found, we investigated whether Magnetic Resonance Imaging (MRI) of the breast could detect a clinically and mammographically occult breast tumor. MRI detected an enhancing lesion in 3 women and an enhancing double lesion in one patient. MRI directed ultrasound guided fine needle aspiration cytology confirmed the 5 breast carcinomas in the 4 women. In women with metastasis in an axillary lymph node consistent with breast ca… Show more

“…In our patients with axillary lymph node metastases who presented with negative findings in mammographic and physical examinations, primary breast cancer was detected in 75-86% by MRI compared to the 80% detection rate reported by Tilanus-Linthorst et al [20] . Moreover, the incidence of an axillary lymph node manifestation from an occult primary breast cancer was low, with rates ranging from 0.3 to 0.8% of all patients with carcinoma of the breast at the time of presentation as reported by Solin et al [21] .…”

“…Sensitivity is not impaired by dense parenchyma. Moreover, the technique is known to detect breast cancers that are undetected at mammography even when the breast tissue is not dense [37,38]. In addition, MRI does not employ ionizing radiation.…”
