Multicenter Case-Control Study of Risk Factors for Histoplasmosis in Human Immunodeficiency Virus-Infected Persons

Abstract: We conducted a multicenter case-control study to identify risk factors for histoplasmosis among persons with acquired immunodeficiency syndrome (AIDS) and to evaluate predictors of a poor outcome (defined as death or admission to the intensive care unit). Patients with histoplasmosis were each matched by age, sex, and CD4 lymphocyte count to 3 controls. From 1996 through 1999, 92 case patients and 252 controls were enrolled. Of the case patients, 81 (89%) were men, 50 (55%) were black, 78 (85%) had a CD4 lymph… Show more

“…In French Guiana, most human activities are practiced during the dry season and therefore are conducive to greater exposition to histoplasmosis molds. Furthermore, the role of the dry season as an activator of the development/aerial dispersion of mold is plausible [6][7][8][9].…”

“…It should be noticed that skin lesions are seldom observed in North American patients (8,9). Virulence and tropism of the isolates predominant in the sub-continents may explain these differences (8) although late diagnosis in Latin American countries may also explain the more extensive dissemination observed in these patients.…”

“…Disseminated histoplasmosis was later included in the list of opportunistic infection defining AIDS. Despite the increased use of highly active antiretroviral therapy (HAART), histoplasmosis remains an important opportunistic infection among HIV patients in endemic areas and frequently appears as the first manifestation of HIV infection or as the AIDS-defining illness (6)(7)(8)(9).…”

“…A high proportion of skin manifestations had been previously reported in studies conducted in Latin American countries such as French Guyana (13%), Panama (17%), and Brazil (43-66%), [9][10][11]14,18,30,31 in contrast to much lower rates reported among patients in the United States (1-7%). 28,29,37 Karimi and others 11 have proposed an association between these skin and mucosal lesions and genetic variations occurring in the etiological agent infecting the patient and suggest that more studies are needed to further evaluate this observation.…”
