Acinetobacter baumannii is one of the most challenging pathogens, on account of its predisposition to develop resistance leading to severe, difficult-to-treat infections. As these bacteria are more usually isolated from nosocomial infections, the new therapeutic options are demanded. Antimicrobial peptides (AMPs) are compounds likely to find application in the treatment of A. baumannii. These compounds exhibit a wide spectrum of antimicrobial activity and were found to be effective against biofilm. In this study, eight AMPs, namely aurein 1.2, CAMEL, citropin 1.1., LL-37, omiganan, r-omiganan, pexiganan, and temporin A, were tested for their antimicrobial activity. A reference strain of A. baumannii ATCC 19606 was used. Antimicrobial assays included determination of the minimum inhibitory concentration and the minimum biofilm eradication concentration. Considering the fact that the majority of A. baumannii infections are associated with mechanical ventilation and the use of indwelling devices, the activity against biofilm was assessed on both a polystyrene surface and tracheal tube fragments. In addition, cytotoxicity (HaCaT) was determined and in vitro selectivity index was calculated.

Introdução: as meningites representam importantes quadros clínicos cujos agentes etiológicos variam, sendo a Acinetobacter baumannii uma bactéria gram-negativa de extrema importância pela sua característica multirresistente e presença elevada em ambientes nosocomiais, afetando principalmente imunossuprimidos e àqueles que passam por procedimentos cirúrgicos, principalmente no que diz respeito a procedimentos neurocirúrgicos. Dessa forma, nos últimos anos tem sido estudado o uso da colistina no tratamento da meningite causada por essa bactéria. Metodologia: para a presente revisão, utilizou-se artigos da plataforma Pubmed, publicados nos últimos cinco anos e com ênfase na temática. Resultados: A colistina é um antimicrobiano criado por volta da década de 1950 que teve seu uso aumentado nos últimos anos por se demonstrar eficaz contra cepas de bactérias multirresistentes, entre elas a A. baumannii. Nesse contexto, a colistina usada por via intratecal ou intraventricular tem sido levantada como alternativa no tratamento de meningites causadas por essa bactéria, uma vez que sua difusão no liquido cefalorraquidiano quando administrada via intravenosa não se mostra satisfatória. Dessa forma, apesar de possuir efeitos adversos como meningite química e ventriculite, possibilidade de contaminação do cateter de aplicação, e ainda uma falta de consenso quanto a dose administrada, o uso do medicamento por essas vias demonstrou eficácia de 89-90% e ausência de efeitos nefrotóxicos experimentados quando há aplicação sistêmica. Conclusão: Assim, o uso da colistina via intratecal ou intraventricular é uma opção vantajosa e eficaz para o tratamento da meningite causada pela A. baumannii multirresistente, no entanto mais estudos são necessários para otimização da terapêutica

The polymyxin antibiotics colistin (polymyxin E) and polymyxin B became available in the 1950s and thus did not undergo contemporary drug development procedures. Their clinical use has recently resurged, assuming an important role as salvage therapy for otherwise untreatable gram‐negative infections. Since their reintroduction into the clinic, significant confusion remains due to the existence of several different conventions used to describe doses of the polymyxins, differences in their formulations, outdated product information, and uncertainties about susceptibility testing that has led to lack of clarity on how to optimally utilize and dose colistin and polymyxin B. We report consensus therapeutic guidelines for agent selection and dosing of the polymyxin antibiotics for optimal use in adult patients, as endorsed by the American College of Clinical Pharmacy (ACCP), Infectious Diseases Society of America (IDSA), International Society of Anti‐Infective Pharmacology (ISAP), Society for Critical Care Medicine (SCCM), and Society of Infectious Diseases Pharmacists (SIDP). The European Society for Clinical Microbiology and Infectious Diseases (ESCMID) endorses this document as a consensus statement. The overall conclusions in the document are endorsed by the European Committee on Antimicrobial Susceptibility Testing (EUCAST). We established a diverse international expert panel to make therapeutic recommendations regarding the pharmacokinetic and pharmacodynamic properties of the drugs and pharmacokinetic targets, polymyxin agent selection, dosing, dosage adjustment and monitoring of colistin and polymyxin B, use of polymyxin‐based combination therapy, intrathecal therapy, inhalation therapy, toxicity, and prevention of renal failure. The treatment guidelines provide the first ever consensus recommendations for colistin and polymyxin B therapy that are intended to guide optimal clinical use.
