Myocardial Performance Index Derived from Preejection Period: A Novel and Feasible Parameter in Evaluation of Cardiac Performance in Patients with Permanent Atrial Fibrillation

Abstract: PEPa-derived MPI had a significant correlation with echocardiographic left ventricular diastolic and systolic function. It may be a novel and feasible indicator in assessment of global left ventricular function in patients with permanent atrial fibrillation.

“…Therefore, we concluded that a higher Em is associated with a better diastolic function. Su et al recently reported that Ea and E/Em were reported to be significantly correlated with time constant of isovolumic LV pressure decline and LV filling pressure in atrial fibrillation patients [14]. In this study, IVRT had a moderately strong correlation with Em and a moderate correlation with E/Em.…”

“…In the present study, we also demonstrated that using the index beat to measure PEPa-derived MPI was as accurate as and less timeconsuming than the onerous method of averaging of multiple cardiac cycles. In addition, our recent study showed that the PEPa-derived MPI measured from average method was significantly associated with left ventricular systolic and diastolic function [17]. The present study demonstrated that PEPa-derived MPI measured by index beat method also had a significant correlation with LVEF and Ea.…”

“…Therefore, using tissue Doppler echocardiography, we can measure PEPa, defined as the interval measured from the onset of QRS to the onset of the systolic mitral annular velocity pattern, ETa, defined as the interval measured from the onset to the end of systolic mitral annular velocity pattern, and IVRTa, defined as the interval measured from the end of systolic mitral annular velocity pattern to the onset of diastolic mitral annular velocity pattern on the same cardiac cycle. Recently, we have demonstrated that the PEPa-derived MPI, defined as the ratio of PEPa + IVRTa to ETa, was significantly associated with echocardiographic left ventricular systolic and diastolic function in AF patients [17]. Because the PEPa-derived MPI can concurrently reflect left ventricular systolic and diastolic function, it has a potential to become a useful parameter in prediction of adverse CV events in AF patients.…”

“…The MPI, the Tei index, also called the MPI-Tei index, was defined as the ratio of IVCT along with IVRT to ET1, whereas the PEP-derived MPI was defined as the ratio of PEP along with IVRT to ET. 8 …”

“…Preejection period (PEP) can easily be obtained in AF patients, and PEP may be used instead of IVCT in AF patients. 8 PEP interval was measured from the onset of QRS to the onset of the systolic mitral annular velocity pattern. Su et al 9 defined the “PEPa-derived MPI” in patients with AF; it is calculated as the ratio of PEPa along with IVRTa to ETa.…”
