Myxedema Coma: A New Look into an Old Crisis

Abstract: Myxedema crisis is a severe life threatening form of decompensated hypothyroidism which is associated with a high mortality rate. Infections and discontinuation of thyroid supplements are the major precipitating factors while hypothermia may not play a major role in tropical countries. Low intracellular T3 leads to cardiogenic shock, respiratory depression, hypothermia and coma. Patients are identified on the basis of a low index of suspicion with a careful history and examination focused on features of hypoth… Show more

“…[74][75][76] Thyroid disorders, both hyperthyroidism and hypothyroidism, can also cause circulatory collapse. 77,78 Pregnancy-associated cardiac conditions, including both peripartum cardiomyopathy and acute coronary dissection, may present as CS. Numerous additional causes of CS have been reported, but they typically occur in <1% of patients.…”

“…4 In addition, the GCS is often relied upon in prehospital research to help control for degree of TBI (for example, see Davis and colleagues 5 ), even though it was not originally intended, nor validated, for this clinical context. Indeed, the GCS is currently being used for non-TBI patients, for instance, to measure brain function in meningitis 6 and hypothyroidism 7 cases. Overall, the GCS has evolved to become a near-universal measure for global mental function, despite its original intent and validation in TBI patients subsequent to stabilization.…”
