Background: The existence of a local allergic rhintis was proposed on the basis of the detection of nasal IgE in the absence of a systemic sensitization. Nevertheless, the significance of this phenomenon remains still unclear.We assessed the presence of mucosal nasal IgE in patients with ascertained allergic rhinitis, nonallergic rhinitis with inflammation and in healthy controls. Methods: Consecutive patients with a well ascertained diagnosis (clinical history, skin prick test, specific IgE assay, nasal endoscopy, nasal cytology) underwent an immunoenzymatic measurement of specific IgE to grass, cypress, parietaria and olive in nasal scrapings.

ÖZAmaç: Bu çalışmada alerjik rinit (AR) ön tanısı veya ayırıcı tanısıyla deri prick testi (DPT) yapılan hastaların alerjen duyarlılığının mevsimsel dağılımı arasındaki ilişki araştırıldı. Hastalar ve Yöntemler:Ocak 2012 -Ocak 2014 tarihleri arasında AR ön tanısıyla kliniğimizde DPT uygulanan 446 hasta (133 erkek, 313 kadın; ort. yaş 33.2±11.8 yıl; dağılım 17-65 yıl) çalışmaya dahil edildi. Pozitif kontrol için histamin hidroklorid (10 mg/dL), negatif kontrol için ise %0.9'luk izotonik NaCl solüsyonu kullanıldı. Bulgular:Deri prick testi 284 hastada negatif iken, 162 hastada pozitif idi. Genel karşılaştırmalarda hastaların sırasıyla en sık akarlara (%42.4), otlara (%8.5) ve koyun yününe (%7.2) duyarlı oldukları saptandı. Deri prick testi sonuçlarının mevsimsel dağılımında, kış mevsiminde sırasıyla akarlar (%43), otlar (%8.6) ve koyun yününe (%7.0) karşı duyarlılık saptanırken, ilkbahar mevsiminde akarlar (%42.6), koyun yünü (%7.6) ve çavdar (%5); yaz mevsiminde akarlar (%33.6), çavdar (%10.6) ve otlar (%10.6) ve sonbahar mevsiminde akarlar (%49.5), otlar (%11.6) ve otlar-tahıl grubuna (%11.6) karşı duyarlılık saptandı. Deri prick testinde otlar -tahıllar ve Cladosporium herbarum'a duyarlılığın mevsimsel dağılımda anlamlı farklılık gösterdiği tespit edildi. Sonuç:Mevsimsel alerjen duyarlılığın DPT ile tespit edilmesi, AR tedavisinin düzenlenmesinde yol gösterici olabilir.Anahtar sözcükler: Alerjik rinit; cladosporium herbarum; mevsim; deri prick testi. ABSTR ACTObjectives: This study aims to investigate the relationship between the results of skin prick test (SPT) and seasonal distribution in patients with preliminary diagnosis or differential diagnosis of allergic rhinitis (AR). Patients and Methods:Between January 2012 and January 2014, a total of 446 patients (133 males, 313 females; mean age 33.2±11.8 years; range 17 to 65 years) who were pre-diagnosed with AR and underwent the SPT at our clinic were included in this study. Histamine hydrochloride (10 mg/dL) was used as a positive control and 0.9% isotonic NaCl solution was used as a negative control.Results: While skin prick test was negative in 284 patients, it was positive in 162. In overall comparison, positive response was determined in the SPT most frequently against house dust mites (42.4%), grass pollens (8,5%) and sheep wool (7.2%), respectively. In the seasonal distribution of SPT results, we found sensitivity most frequently against house dust mites (43%), grass pollens (8.6%) and sheep wool (7.0%), respectively in winter; against house dust mites (42.6%), sheep wool (7.6%) and rye (5%), respectively in spring; against house dust mites (33.6%), rye (10.6%) and grass pollens (10.6%), respectively in summer; and against house dust mites (49.5%), grass pollens (11.6%) and grains (11.6%), respectively in autumn. Positive responses of the grains and Cladosporium herbarum in the SPT were significantly different in seasonal distribution. Conclusion:The identification of seasonal allergen sensitivity by SPT may be a guide to regulate AR treatment.
