Management of bilateral Wilms' tumor is particularly challenging, considering the chances of recurrence and long-term renal function for affected patients. Aggressive surgical resection to prevent recurrence must be balanced with the desire to preserve renal function. We evaluated our experience in the management of bilateral Wilms' tumor stressing the challenges encountered in decision making and the role of nephron sparing surgery. We had four children presenting with bilateral Wilms' tumor. All of them were appropriately staged and given standard chemotherapy as per NWTS-5 guidelines. Tumors were considered to have a 'good' response to chemotherapy if sufficient tumor shrinkage was observed so that renal hilum was seen free of tumor and vice versa. Nephron-sparing surgery was considered in all and was performed when feasible, followed by completion adjuvant chemotherapy. All patients were followed up with serial ultrasound scans (3-6 monthly) and CECT abdomen (yearly once). Blood urea and serum creatinine, hypertension, and proteinuria were assessed during follow-up visits. All four children received neo-adjuvant chemotherapy as per NWTS-5 guidelines. The first child had poor response to chemotherapy and was considered for left radical and right partial nephrectomy. However, patient attenders refused any surgical intervention and the child was taken home. The second child had a 'good' response on left side and was planned for left partial and right radical nephroureterectomy based on pre-operative imaging analysis. However, intra-operatively, bilateral partial nephrectomy with good margins was feasible. The third child though, showed 'poor' response to pre-op chemotherapy and warranted bilateral nephroureterectomy, right partial and left radical nephroureterectomy was feasible. However, in the fourth child, we were not able to perform nephron sparing surgery and left nephroureterectomy with right tumor biopsy was done. Following this, child was started on 2nd-line chemotherapy, now awaiting right partial nephrectomy. The second child is on follow up for 1.5 years, doing well. However, the third child expired 1.5 years following surgery due to recurrence (lung metastasis). Management of bilateral Wilm's tumor is challenging and nephron-sparing surgery should be considered in all patients having bilateral Wilm's tumor with favorable histology, even if pre-operative imaging studies suggest that the lesions are unresectable.

Objectives: We investigated the relationship between the surgical margin in partial nephrectomy (PN) and thymidine phosphorylase (TP)-expressing macrophages in peritumoral tissue of renal cell carcinoma (RCC). Methods: In 46 patients who underwent radical nephrectomy, we measured TP protein levels in tumor tissue, peritumoral tissue and normal tissue, and conducted immunohistochemical staining for TP and macrophages. In addition, we prospectively conducted PN with a 5-mm margin in 11 patients with pT1a RCC. Results: The TP protein level and TP-positive macrophages were correlated with T classification, histological grade, mode of infiltration and venous invasion. However, for pT1 RCC, TP-positive macrophages in pT1a were significantly lower than in pT1b (p = 0.0140), while there was no significant difference in TP protein levels between pT1a and pT1b. No surgical margin was positive in 11 patients who underwent PN with a 5-mm margin, and no patient had local recurrence or distant metastasis during follow-up. Conclusions: The TP protein level and TP-positive macrophages in the peritumor area are thought to be associated with tumor progression in RCC, while a similar relationship was not found in pT1a RCC. These data suggest that a 5-mm margin might be safe to reduce the risk of local recurrence when PN is performed for treatment of solitary pT1a RCC.

Įvertinti inkstų rezekcijų, atliktų dėl įvairaus dydžio inkstų T1a stadijos vėžio, funkcinius ir onkologinius rezultatus bei apžvelgti juos tarptautinės patirties kontekste. Tyrimo medžiaga ir metodika. Atliktas retrospektyvinis tyrimas, kurio metu buvo analizuoti visi inkstų rezekcijos atvejai (n = 332) 14 metų laikotarpiu. Į galutinę analizę pateko tik T1a stadijos inkstų navikai (n = 179). Atrinkti atvejai suskirstyti į tris grupes pagal didžiausią išmatavimą (cm): A (<2), B (2–2,9), C (3–4). Vertinti funkciniai, onkologiniai rezultatai ir bendras pacientų išgyvenamumas. Statistinė analizė atlikta SPSS 16.0 programa (statistiškai reikšminga, kai p < 0,05). Rezultatai. Vertinant histologinius navikų duomenis, daugiausia operuota šviesių ląstelių karcinomos atvejų 146 (81,6 proc.), papilinės karcinomos – 23 (12,8 proc.), chromofobinė karcinoma – 10 (5,6 proc.). Pagal diferenciacijos laipsnį daugiausia pastebėta G2 (vidutiniškai diferencijuotų) navikų – 109 (62,3 proc.), 52 (29,7 proc.) atvejais nustatytas G1 (gerai diferencijuotas) navikas, G3 (blogai diferencijuoti) navikai rasti 14 (8 proc.) pacientų. Pagal Fuhrman klasifikaciją, I laipsnis pastebėtas 28 (18,9 proc.) atvejais, 83 (56,1 proc.) – II laipsnis, 36 (24,3 proc.) – III laipsnis, 1 (0,7 proc.) – IV laipsnis. Diferenciacijos laipsnio pasiskirstymo skirtingose dydžių grupėse reikšmingo skirtumo nepastebėta. Ūminis inkstų nepakankamumas išsivystė 90 (71,4 proc.) iš tirtų 126 pacientų, iš kurių 15 buvo atlikta vienintelio inksto rezekcija (93,8 proc. visų vieno inksto rezekcijų). Vertinant inkstų funkcijos rodiklius, A ir C grupėse vėlyvame pooperaciniame periode glomerulų filtracijos greitis beveik pasiekė priešoperacinį lygį, B grupėje jis išliko žemesnis. 1, 5 ir 10 metų išgyvenamumas be ligos recidyvo buvo 99,2 proc. ir 99,7 proc. ir 86,4 proc., bendras išgyvenamumas atitinkamai – 98,3 proc., 88,9 proc. ir 83,7 proc. Medianinė pacientų stebėjimo trukmė 54 ± 36 mėn. Išvados. Inkstų rezekcijos dėl T1a navikų yra saugios operacijos, padedančios išsaugoti inkstų funkciją bei pasižyminčios gerais onkologiniais rezultatais. T1a navikų grupė nėra vienalytė. Naviko dydis yra svarbus prognozuojant naviko diferenciacijos laipsnį, tikėtiną operacijos trukmę bei galimų recidyvų dažnį. Į tai reikėtų atsižvelgti renkantis radikalų inkstų vėžio gydymą. Šios operacijos taip pat padeda išsaugoti inkstų funkciją, ypač vienintelio inksto navikų atveju.
