“…There are reports which suggest that headache associated with the above mentioned signs can be a usual presentation of CCF, in up to 40-50% cases [10,11], but rare are those which point out headache as the only clinical symptom of CCF. These, CCF associated headaches, in small number of cases, can be presented as mimicking primary headaches [12], but in our patients it was a rare clinical phenotype of primary headache, NDPH. This type of headache is one of the most common and often difficult headache disorders for diagnosis and therapy.…”

“…On the other side, it must be borne in the mind that the absence of abnormalities on noninvasive imaging studies (as was in initial CT scanning in here presented case) does not exclude the diagnosis of CCF [13]. Thus, cerebral angiography presents the gold standard imaging modality in the diagnosis of CCF, which is useful in identifying the localization of fistula, evaluating venous drainage and assessing collateral circulation [12,13]. While low risk CCF with mild symptomatology can be managed conservatively, higher risk fistulas deserve the most aggressive approach in order to eradicate the fistula.…”

“…The most typical presentation of NDPH is the acute onset headache on a specific day which becomes persistent within 24 hours. Most NDPH patients can recall the day when their headaches started (7,17,24). Hence, in the latest version of the ICHD, clearly remembering the onset is listed as a diagnostic criterion (9).…”

“…Headache features are not restricted, and migrainous features may be more common than TTH features (7,8,22,25,35), reflecting the evolution of the diagnostic criteria. Notably, despite the prominent migrainous features, most patients presented with bilateral headache that is commonly pressing/tightening (7,21,22,24). Previous headache disorders may exist in up to 29% of the patients with NDPH (7,8,17,22,24), but a chronification of preexisting headache disorders or de novo NDPH must be carefully differentiated based on the headache features changes, if any, and pre-NDPH headache frequency.…”

“…Perhaps NDPH is a heterogeneous disease entity, consisting of both primary and secondary etiologies. More specifically, as some have suggested, that NDPH is rather a syndrome than a distinct disease [ 11 ]. In the current review, we will focus on the progress of our understanding of NDPH over the last few years.…”
