“…Complex forms of transposition of the great arteries (TGA) or double outlet right ventricle (DORV) accompanied by left ventricular outflow tract (LVOT) obstruction were rarely amenable to anatomic biventricular repair until the introduction of the Rastelli operation. 1 Although a good solution in most such cases, it has some shortcomings: The left ventricle-aortic valve connection is often curved and vulnerable to stenosis; the right ventricular outflow tract (RVOT) reinterventions are common due to the use of a right ventricle to pulmonary artery (RV-PA) conduit; in addition, it cannot be done when remote/small ventricular septal defect (VSD) or a straddling atrioventricular valve is present. 2 Another option is the R eparation a l'Etage Ventriculaire (REV), but still with the need for an LVOT tunnel, although less prone to stenosis.…”

“…This has led to other techniques aiming to restore anatomy closer to the normal biventricular circulation. The Nikaidoh procedure (aortic root translocation [ART]) 1 aligns the LVOT and the aorta; only the aortic annulus is preserved and RVOT reconstruction is with an RV-PA conduit or patch. Likewise, the en bloc rotation (EBR) of the outflow tracts or double root rotation modification 3,4 creates a straight systemic ejection pathway and preserves at least part of the pulmonary annulus and its growth potential.…”

“…The Nikaidoh operation 1,2 shows excellent early results; however, long-term changes in the aortic wall remain undefined. We have recently reported the structure and function of the aortic and pulmonary outflows in a patient 12 years after the Nikaidoh operation 3 .…”
