Nipple-Sparing Mastectomy

Abstract: The incidence of partial nipple necrosis was high and was related to circumareolar incisions. Most cases of nipple necrosis are superficial and heal uneventfully. Preservation of the nipple improves the aesthetic outcome of immediate breast reconstruction.

“…Although the incision types vary with configuration, the impasse of the access incision with no complications has drawn attention in the great part of the studies [20,[25][26][27][28]30,33] (Figure 1). A critical survey shows that the procedure is normally performed by numerous approaches, but the greater part more than one type of incision is performed [11][12][13][14][15][16][18][19][20][21][22]30,[33][34][35][36][37]39,59,61,63,[66][67][68][69] . In fact, Endara et al [39] analyzed 48 NSM studies, of which 41 described details related to the type of NSM incision.…”

“…NSM is an increasingly popular alternative to skin-sparing mastectomy (SSM), as it allows for preservation of the nipple-areolar complex (NAC) [1, 3, 4]. With proper patient selection, NSM can be used in both the prophylactic and the therapeutic settings [1, 4–6].…”

“…With proper patient selection, NSM can be used in both the prophylactic and the therapeutic settings [1, 4–6]. Regardless of the indication, the central tenets of NSM are to remove the glandular breast tissue while maximizing structural preservation of the breast and adhering to oncologic standards [1, 3]. The trend towards the development of more advanced NSM modifications is driven by patient demand and an increasing amount of literature documenting its therapeutic success [7].…”
