Abstract: Central mass-like regeneration appears to be a common regenerative pattern in cholangiopathic disorders and should not be mistaken for malignancy.

“…All masses (nodules) in previously described similar pediatric cases have been lesions larger than 5 cm [16]. In our case, the size of the nodule (7 × 5 cm) is compatible with that in previous reports [1,16,[18][19][20][21].…”

“…In ALGS hepatic nodules display less fibrosis and relatively well preserved interlobular bile ducts than the rest of the liver that shows either extensive fibrosis or liver cirrhosis [19]. In our case, the nodule was seen adjacent to the right portal vein, was located in the central zone of the liver, as in previous reports [1,16,[18][19][20][21], and revealed relatively well preserved interlobular bile ducts without fibrosis, whereas the rest of the liver or non-nodular lesion revealed paucity of bile ducts with stage 1 hepatic fibrosis.…”

“…Wenzel et al [50] speculated that it contributes to periportal fibrosis or apoptosis. Giant nodular fibrosis in the central liver was similar to regenerative nodules, described as a regenerative pattern led by cholestasis [51]. Patchy fibrosis has been described in two previous case reports [11,28].…”

“…What's more, to our surprise, the giant nodule in the liver adjacent to the right portal venous was recognized as hepatic regenerative nodule according to imaging and pathology. Few previous literatures reported, this may be considered a benign characteristic with typical location and vessels coursing through [3,4]. Comparative analysis of pathology features between the nodule and remainder of the liver parenchyma showed less damaged liver and visible interlobular bile ducts within the mass with unknown mechanism.…”
