Non-Exposed Endoscopic Wall-Inversion Surgery for Gastrointestinal Stromal Tumor of the Stomach: First Case Report in Korea

Abstract: Laparoscopic wedge resection of the stomach is a widely accepted treatment for primary resectable gastrointestinal stromal tumors (GISTs). However, it is difficult to determine the appropriate incision line from outside of the stomach, and many attempts have been made to avoid unnecessary resection of unaffected gastric tissues. Recently a technique called non-exposed endoscopic wall-inversion surgery (NEWS) was introduced to avoid exposure of GIST to the peritoneum. Here, we describe the first published case … Show more

“…Thus a more secure endoscopic method is needed, and it should be performed by a well-trained endoscopist. Recently, novel hybrid techniques, such as combination of laparoscopic and endoscopic approaches to neoplasia with non exposure technique (CLEANNET) (76) and non-exposed endoscopic wall-inversion surgery (NEWS) (77,78), could avoid exposing malignant SETs to the peritoneal cavity. In conclusion, technical modifications and improvements are required to define the role of endoscopy for treating gastric GISTs.…”

“…NEWS is a novel technique developed by Goto et al ( 6 ) that consists of a minimally invasive procedure using laparoscopic surgery and endoscopic intervention with a full-thickness resection of the gastric wall and a decreased contamination risk. NEWS has been used in patients with schwannoma, GISTs, granuloma, an ectopic pancreas and neurinoma ( 7 , 12 , 13 ), and has been combined with sentinel node basin dissection for early stage gastric cancer ( 8 9 ).…”

“…Finally, circumferential muco-submucosal incision of the intruded tumor is performed by endoscopists and tumor specimen is retrieved perorally followed by mucosal closure ( 80 , 181 , 194 ). Besides the benefit that precludes interluminal communication, NEWS could achieve an accurate determination of the resection line ( 195 ). However, it is obvious that the NEWS technique is restrained by tumor size as tumors with a relatively larger size (>3 cm) are laborious to be retrieved through the mouth.…”
