Noninvasive Ventilation Intolerance: Characteristics, Predictors, and Outcomes

Abstract: BACKGROUND: Noninvasive ventilation (NIV) intolerance is one reason for NIV failure. However, the characteristics, predictors, and outcomes of NIV intolerance are unclear. METHODS: A prospective observational study was performed in the respiratory intensive care unit of a teaching hospital. Subjects with acute respiratory failure who used NIV were enrolled. Initially, continuous use of NIV was encouraged. However, if the subject could not tolerate NIV, it was used intermittently. NIV intolerance was defined as… Show more

“…The patient tolerance scale in the helmet group was remarkably higher than in the facial mask control group [14, 17, 29, 39]. Better tolerance allowed a longer continuous application [14, 31] and increased the success rate of NIV [40]. In fact, no patients failed NIV as a result of claustrophobia, discomfort, or pain related to a helmet while 38 % patients might have a failed NIV owing to a mask [14], which would increase hospital mortality.…”

“…9,11 The main explanation for this finding may be the mild severity in these subjects compared with subjects in previous studies. Because decreased severity is associated with better tolerance, the low severity of symptoms in the subjects included by Liu et al 13 could explain the low prevalence of poor tolerance in their study. As expected, the authors reported a poorer outcome in subjects with intolerance, since subjects experiencing NIV intolerance were more frequently intubated (44% vs 26%, P ϭ .008) and presented a trend toward higher mortality (34% vs 22%, P ϭ .08).…”

“…The study by Liu et al 13 has a number of limitations. First of all, the authors did not look for the presence of patient-ventilator asynchrony.…”
