Novel technique using the Outback® LTD™ catheter for a common iliac artery occlusion

Abstract: We present a case of an unusual arterial--arterial anastamosis that resulted from an unsuccessful attempt at crossing a total occlusion of the common iliac artery. Subsequently, we were able to successful recanalize the artery using a modified technique with the Outback reentry catheter.

“…This device has been commonly used for femoral or popliteal arteries; however, it has been increasingly used for the iliac artery . Recently, several reports have documented use of re‐entry puncture at the distal aorta to achieve recanalization of total occlusion of the CIA . Because of the risk of device puncturing outside of the aorta, re‐entry catheter usage and subsequent ballooning had a risk of major bleeding.…”

“…However, data on subintimal revascularization of chronic iliac artery occlusions using the Outback catheter are sparse. Mixon et al published a feasibility report about subintimal revascularization of an iliac artery in one patient with no follow-up data [16]. Ramjas et al performed recanalization of chronic common iliac artery occlusions by means of an Outback catheter in a series of eight patients [8].…”
