Number Needed To… $ave?

Abstract: The 'Number Needed to Treat' (NNT) is a useful measure for estimating the number of patients that would need to receive a therapeutic intervention to avoid one of the adverse events that the treatment is designed to prevent. We explored the possibility of an adaption of NNT to estimate the 'Number Needed to $ave' (NN$) as a new, conceptual systems metric to estimate potential cost-savings to the health system from implementation of a treatment, or in this case, a program. We used the outcomes of the INSPIRED C… Show more

“…A recent economic analysis of the costs and outcomes of the INSPIRED program estimated that only 26 patients need to complete the program to avert an estimated $100 000 in hospital-based costs. 19 Of 84 patients enrolled in the program who died from 2011 to 2015, 32 (38%) were supported in dying at home through effective advance care planning (current completion rate of written personal directives is > 80%). 20 A mixed-method evaluation of the INSPIRED program confirmed high patient satisfaction and the program's continued impact in reducing emergency department visits, hospital admissions and days in hospital by more than 60% among participants.…”

“…The pan-Canadian INSPIRED COPD collaborative supported 19 teams across Canada, successfully adapting INSPIRED. The experience makes the point that champions, enthusiasm, patient, and caregiver participation in design and delivery of evidence-based practices in a feasible approach within the community, coupled with insightful investment in change, 8 can triumph over forces of negativity that pervade our traditional healthcare systems. With more than 1000 patients enrolled across Canada (as of September 2016 and in addition to the ∼500 enrolled in Halifax), several teams have already demonstrated similar outcomes to the Halifax initiative, and over the next few months, we will gather outcomes that matter both to patients and to those with funding responsibilities.…”
