Nurse‐led supportive Coordinated Transitional Care (CTraC) program improves care for veterans with serious illness

Abstract: BackgroundThe Coordinated Transitional Care (CTraC) program is a telephone‐based, nurse‐driven program shown to decrease readmissions.
