Introduction Recognizing the values and norms significant to healthcare organizations (Safety Culture) are the prerequisites for safety and quality care. Understanding the safety culture is essential for improving undesirable workforce attitudes and behaviours such as lack of adverse event reporting. The study assessed the frequency of adverse event reporting, the patient safety culture determinants of the adverse event reporting, and the implications for Ghanaian healthcare facilities. Methods The study employed a multi-centre cross-sectional survey on 1651 health professionals in 13 healthcare facilities in Ghana using the Survey on Patient Safety (SOPS) Culture, Hospital Survey questionnaire. Analyses included descriptive, Spearman Rho correlation, one-way ANOVA, and a Binary logistic regression model. Results The majority of health professionals had at least reported adverse events in the past 12 months across all 13 healthcare facilities. Teamwork (Mean: 4.18, SD: 0.566) and response to errors (Mean: 3.40, SD: 0.742) were the satisfactory patient safety culture. The patient safety culture dimensions were statistically significant (χ2 (9, N = 1642) = 69.28, p < .001) in distinguishing between participants who frequently reported adverse events and otherwise. Conclusion Promoting an effective patient safety culture is the ultimate way to overcome the challenges of adverse event reporting, and this can effectively be dealt with by developing policies to regulate the incidence and reporting of adverse events. The quality of healthcare and patient safety can also be enhanced when healthcare managers dedicate adequate support and resources to ensure teamwork, effective communication, and blame-free culture.

Introdução: Os eventos adversos implicam dano ou lesão, podendo acarretar até mesmo a morte do paciente, e embora os eventos com dano não sejam reservados ao exercício da enfermagem, configuramos para essa discussão foco nos eventos presentes nas ações da categoria. Objetivo: Analisar as evidências científicas sobre a ocorrência de eventos adversos associados às práticas assistenciais de enfermagem. Métodos: Revisão integrativa com busca nas bases de dados Medline, Embase, Lilacs, Cinahl e Web of Science, a partir dos descritores em ciências da saúde drug-related side effects and adverse reactions, adverse drug reaction, side effects, medication errors, team nursing, nursing care. Resultados: Foram selecionados oito estudos para compor a análise da ocorrência dos eventos adversos na enfermagem. Evidenciaram-se cinco principais eventos adversos associados às práticas de enfermagem, relacionados à medicação, quedas, lesões por pressão, infecções associadas aos cuidados em saúde e perda acidental de dispositivos de saúde. Conclusão: As evidências reconhecem que a maioria dos eventos adversos revelados na busca foram classificados como evitáveis, ou seja, passíveis de prevenção por meio da adoção de medidas mais seguras.

Introduction: The use of totally implantable venous access port systems (TIVAPS) is crucial for long-term intravenous treatment in children and adults patients. The purpose of this article is to examine the extent of existing research on complications related to implantation and management of TIVAPS. Methods: A scoping review was performed from September to December in 2019. The MEDLINE, CINAHL and PubMed databases were used to review and analyze the literature using various combinations of keywords and their English-language synonyms. Quantitative clinical research published from 2010 to December 2019 were included in the analysis. The PRISMA-ScR recommendations were followed. Results: Out of the 219 search results, 26 studies were included in the final analysis. To identify complications associated with the implantation and handling of TIVAPS, free codes were identified by thematic analysis leading to the development of 17 primary grade descriptive subthemes. For the development of the thematic framework, all secondary level sub-topics were synthesized into three main themes: time-bound complications, infection-related complications, and TIVAPS removal complications. Discussion: Nurses who use TIVAPS on children and adult patients should be aware of the possible complications associated with TIVAPS. Refinement of surgical techniques and improving care process may improve the longevity of the line. Proper and careful management with TIVADs and more frequent observation by nurses are likely to have positive consequences for patients and positive cost implications for the hospital.
