Nursing Within Primary Care Settings in Atlantic Canada: A Scoping Review

“…[1][2][3][4] Registered nurses have been instrumental in improving care quality in ambulatory care settings [5][6][7] ; however, how RN roles are operationalized in these settings remains unclear. 8,9 Implementation of team-based models of care such as the patient-centered medical home (PCMH) has led to a greater emphasis on defining the role of the RN in ambulatory settings. The Veterans Health Administration (VA) launched a national rollout of the PCMH called Patient Aligned Care Teams (PACTs) to more than 150 medical centers and 800 community-based outpatient clinics starting in 2010.…”

“…At the meso- level, organizations must create institutional policies and infrastructure allowing for optimal NP function (Phillips, 2022; Torrens et al, 2020). Health professional colleagues at the micro -level must have strong working relationships with NPs, and patients must understand the NP role (Curnew & Lukewich, 2018; DiCenso et al, 2007; Poghosyan et al, 2020). Although macro -level policies may act as facilitators more so than barriers in nations with advanced levels of integration (e.g., United States and Canada), meso -level and micro- level policies may require attention in organizations in any country newly introducing the NP role.…”

“…Organisational attributes can influence nurse and patient outcomes, influencing the extent to which nurses enact the full scope of their role. Recent systematic and scoping reviews have examined the role of nurses in primary care settings; 4–7 however, only one review 6 has focused specifically on well-child care. Furthermore, a recent review summarised tools that measure the influence of organisational attributes on nursing contributions to chronic disease management; 8 however, no review has specifically examined organisational attributes that influence nursing contributions to well-child care.…”
