Introduction: During the intensive care unit (ICU) stay, the patient has caloric sources that do not come from nutrition, such as propofol. When these calories are not counted, it becomes a probable source of error in the critical patient’s energy calculation and it is already known that overfeeding is associated with many complications, including mortality. Our study aimed to assess the relevance of calories from propofol sedation in 51 mechanically ventilated intensive care (ICU) patients with COVID-19 receiving enteral nutrition. Methods: This is a retrospective study. Results: A total of 51 patients were recruited, 76% of patients receiving propofol during hospitalization. Among these, the average amount of calories from propofol was 260 (IQR 20-594) kcal. The proportion of calories in propofol was 15% (SD ± 8) of the total caloric value and the mean time of use was 8 days (SD ± 5.5). Conclusion: The conclusion we reached was that the calories from propofol were not relevant to exceed the proposed energy goal and increase the risk of hyperalimentation. However, in individual cases, the amount of calories can reach important values.

Objective: To explore the therapeutic effects of early enteral nutrition (EEN) on patients with sepsis on mechanical ventilation. Methods: Patients with sepsis on mechanical ventilation in the medical intensive care unit (ICU) from January 2013 to March 2016 were treated with enteral nutrition. Patients treated within 48 hours of initiation of mechanical ventilation were assigned to the EEN group, and the rest were assigned to the delayed enteral nutrition (DEN) group. Peripheral blood Th17 cells and Treg cells, endotoxin (ET) level, 28-day mortality, duration of mechanical ventilation, lengths of ICU stay and hospital stay, and incidence of ICU-acquired weakness (ICU-AW) were analyzed between the 2 groups. Results: The proportion of Th17 cells and ET levels in the EEN group were significantly lower than those in the DEN group, whereas the proportion of Treg cells in the EEN group was remarkably higher than that in the DEN group ( P < .05). The duration of mechanical ventilation, lengths of ICU stay and hospital stay, and incidence of ICU-AW were higher in the DEN group than in the EEN group ( P < .05), but there was no significant difference in the 28-day mortality between the 2 groups. Conclusion: Patients with sepsis mainly present with an increased proportion of Th17 cells in the early stage, manifesting as enhanced immune response. Early enteral nutrition can inhibit the excessive immune response, shorten the duration of mechanical ventilation, lengths of ICU stay and hospital stay, and reduce the incidence of ICU-AW, but it has no obvious effect on 28-day mortality.

Knowledge levels and practices of intensive care nurses regarding enteral nutrition in patients receiving inotropic treatment: a descriptive, cross-sectional study

Objective This study aimed to investigate the level of knowledge of intensive care nurses about enteral nutrition of patients receiving high-dose inotropic therapy in the intensive care unit. Methods This descriptive and cross-sectional study collected data from 302 nurses working in intensive care units of a teaching and research hospital between April and July 2022. The data were collected using the Sociodemographic Information Form and the Enteral Nutrition Knowledge and Practices Questionnaire. Results In the study, it was found that nurses mostly gave correct responses to the questions about the ways of administration of enteral nutrition (99%), complications of enteral nutrition (97.7%), follow-up of complications (97.7%), and evaluation of nutritional tolerance in patients with decreased inotrope requirement (94.4%). It was seen that the least correct responses were given to the questions related to the follow-up of gastric residual volume in patients receiving inotrope therapy (34.4%) and the feeding rate until hemodynamic stability was achieved in patients with decreased inotrope requirement (39.7%). A statistically significant result found that nurses with intensive care certificates had better knowledge of enteral nutrition (p<0.01). Conclusion The study found that intensive care nurses' knowledge of enteral nutrition practices was at a good level. In contrast, their knowledge of the nutritional practices of patients receiving inotropic therapy should be improved.
