Nutritional aspects in patient undergoing liver resection

Abstract: In the past two decades, hepatic surgery has achieved important technical breakthroughs resulting in a drastic reduction of the onset of complications and in an improved post-resective survival. Pre-operative nutritional status is one of the key points for the success of a liver resection. Modern surgical achievement such as the development of living-related liver donation, and the possibility to perform more laparoscopic liver resection gave us the opportunity to extend post-operative protocol focused on earl… Show more

“…The CgA levels in the Barcelona staging of HCC was higher in stage D compared to stage C (p<0.01), to stage B (p<0.001), and to stage A (p<0.001). The most frequent site of neuroendocrine tumours is the gastrointestinal tract, accounting for approximately 70% [15] of the total neuroendocrine tumours in the body [16,17]. Neuroendocrine tumours frequently metastasize to the liver, but the liver itself seldom is the site of a primary tumour [18-20].…”

“…Глюконеогенез, происходящий в печени, потребляет до 40-60% лактата. В случае повреждения печени, она вырабатывает лактат, а не метаболизирует его [103].…”

“…In fact, there is relatively weak evidence to support the practice of carbohydrate loading in advance of surgery, but afterward it is clear that early introduction of enteral nutrition can reduce overall morbidity and specifically septic complications (39)(40)(41)(42). Following hepatic resections, patients are in a catabolic state characterized by glucose and electrolyte imbalances mitigated by increased metabolic demand (43). Some surgeons advocate the use of glucose containing fluids in the post-resection resuscitation algorithms in order to abrogate the catabolism.…”
