OBJETIVO: Definir o estado nutricional e estabelecer a prevalência de deficiência de vitamina A (DVA) em puérperas atendidas em maternidade pública brasileira, além de avaliar a influência de características maternas e obstétricas sobre os níveis de retinol no soro e no colostro nessa população. MÉTODOS: Participaram do estudo de corte transversal 97 puérperas saudáveis classificadas quanto à idade, estado nutricional, paridade, via de parto, peso ao nascer e idade gestacional do recém-nascido. Três amostras de colostro e uma de soro foram coletadas em jejum no pós-parto imediato. O retinol foi analisado por cromatografia líquida de alta eficiência. Para caracterizar o estado nutricional em vitamina A, foi adotado ponto de corte para retinol no soro de 30µg/dL e, no colostro, de 60µg/dL. RESULTADOS: No grupo total de puérperas, a concentração média de 60µg/dL de retinol no colostro e 43µg/dL no soro indicou estado bioquímico adequado. No entanto, ao se avaliar individualmente, constatou-se alta prevalência de DVA subclínica no soro (15%) e no colostro (50%). Não foi verificada influência das características maternas, obstétricas e do neonato sobre os níveis de retinol no soro e no colostro das mulheres (p>0,05). CONCLUSÕES: O risco de DVA pode ocorrer em gestantes/puérperas independentemente das características maternas e obstétricas consideradas. Esse fato reforça a necessidade da atenção especial ao acompanhamento pré-natal de todas as mulheres, a fim de prevenir a instalação da DVA e garantir a redução dos índices de morbimortalidade infantil e materna.

Background/Aims: Assessing the diet and biochemical indicators of vitamin A deficiency (VAD) in high-risk populations is crucial in cases where this deficiency is mainly caused by chronically inadequate intake. This study aimed to determine the retinol and betacarotene status in mother-infant dyads, and to evaluate the associations between them. Methods: Umbilical cord serum, maternal serum, and colostrum were collected from 134 healthy mothers living in a risk region for VAD. Vitamin A and betacarotene were quantified by liquid chromatography, and dietary information was collected using a food frequency questionnaire. Results: Although the overall mean intakes of vitamin A and betacarotene were considered adequate, 16% of the women had insufficient intake. Mean retinol levels were also adequate, yet low levels were diagnosed in about 8% of the mothers, based on maternal serum and colostrum, and in 16% of the cord serum samples. Retinol and betacarotene were positively associated in cord serum (p = 0.004), maternal serum (p = 0.041), and colostrum (p < 0.001) but was not associated with dietary intake. Conclusions: A diagnosis of adequacy based on mean biochemical and dietary data of this population in fact masks the marginal vitamin A status presented by mothers and children.

Background Vitamin A deficiency is still considered to be a nutritional problem during pregnancy, lactation and early childhood. The present study aimed to assess the vitamin A status of women and their newborns in the Brazilian Northeast and to determine the association between retinol in the maternal serum, umbilical cord blood and colostrum. Methods Vitamin A status in 65 pairs of women and newborns was assessed from samples of the mother’s serum, umbilical cord serum and colostrum using high‐performance liquid chromatography. The inadequacy of the vitamin A status of mothers and infants was identified if the retinol values were <0.7 µmol L−1 in maternal serum or umbilical cord blood or <1.05 µmol L−1 in colostrum. Results The prevalence of inadequate maternal vitamin A status was 21.5% (95% CI: 11.5%–31.5%) and 13.8% [95% confidence interval (CI) = 5.4%–22.2%] based on maternal serum and colostrum, respectively. Among newborns, 41.5% (95% CI = 29.3%–53.5%) presented a low status of vitamin A based on cord serum. Multiple linear regression analysis identified that maternal serum retinol is a predictor of umbilical cord retinol (P = 0.005). Retinol in maternal serum was lower in mothers who were less educated (P = 0.04) and colostrum retinol was higher in older (P = 0.04) and multiparous (P = 0.002) mothers. Conclusions Vitamin A deficiency is a common problem among mothers attended in public hospitals in Northeast Brazil and maternal retinol concentrations are associated with retinol status in newborns. Maternal age, parity and educational level were related to the maternal vitamin A status.
