ObjectiveTo evaluate efficiency of pivot splints in jaw exercises, in combination with stabilization splints, in cases of anterior disc displacement without reduction of temporomandibular joint.Subjects and methodsTwenty-three patients who referred to the prosthodontics clinic in 1995–1997 were included in the study, where anterior disc displacement without reduction of temporomandibular joint was diagnosed using magnetic resonance imaging and clinical examination. Pivot splints were used for jaw exercises for five minutes long; five times/day and stabilization splints were used at all other times. The patients were followed for 24 weeks. Lateral and protrusive excursions along with maximum mouth opening and were evaluated at each control. Bilateral palpation of temporal, masseter, sternocleidomastoid muscles and TMJ was assessed for pain perception before and after treatment. Data were statistically analyzed using Paired sample t-test and Independent Samples t-test (p < .05).ResultsMean mandibular range of motion measurements increased from 28.74 mm prior to 49.17 mm on maximum opening; right/left lateral excursion from 7.61 mm to 12.04 mm and 4.09 mm to 7.3 mm on protrusion after treatment. All changes observed before and after treatment were found to be statistically significant. (p < .001) Pain symptoms were eliminated at the end of 24 weeks of treatment in all patients.ConclusionUsing pivot splints as an exercise regimen along with a stabilization splint may be a viable treatment option for patients with anterior disc displacement without reduction; as normal mandibular range of motion was established and pain was eliminated.

A prospective randomized study was carried out to compare the therapeutic success of two different types of splint in patients with painful anterior disc displacement of the temporomandibular joint. The patients in Group I (n = 20) received stabilization splint therapy and the patients in Group II (n = 20) pivot splint therapy. Clinical investigation of the craniomandibular system was performed before and 1, 2 and 3 months after therapy and this was accompanied by subjective evaluation by the patients of their symptoms, using a validated questionnaire with visual analogue scales (VAS). There was a significant increase in maximum jaw opening and a significant reduction in subjective pain in both groups during the course of therapy (Wilcoxon test, P < 0.05). Active jaw opening increased by a mean of 8.05 mm in the group of patients treated with a stabilization splint (Group I). The comparable figure with pivot splint therapy (Group II) was 8.26 mm. The VAS scale value in Group I was reduced by 30.54 units and in Group II by 39.36 scale units. However, neither of these differences between the groups was statistically significant (Mann-WhitneyU-test, P > 0.05). It can be concluded that both types of splint provided effective therapy in patients with anterior disc displacement.

Manejo de artralgia de articulación temporomandibular mediante la utilización de dispositivos interoclusales tipo pívot; revisión de la literaturaAVANCES EN ODONTOESTOMATOLOGÍA/299 RESUMENArtralgia de la articulación temporomandibular (ATM) es un término utilizado en los criterios diagnósticos para los trastornos temporomandibulares (CD/TTM) definido como dolor de origen articular que se ve aumentado por el movimiento mandibular, ya sea en función o parafunción, y la replicación de este dolor mediante la palpación de la ATM. En relación al manejo de la artralgia, la evidencia describe diferentes tipos de terapias, las cuales se pueden clasificar en invasivas y no invasivas. Las terapias farmacológicas, cognitivo conductuales y el uso dispositivos interoclusales se encuentran dentro de la categoría de terapias no invasivas. Los dispositivos interoclusales tipo pívot, han sido ampliamente utilizados en el alivio de artralgias de ATM. Se describe un 70% de alivio del dolor con el uso de éstos dispositivos en pacientes con artralgias de ATM. El objetivo de esta revisión es evaluar la evidencia encontrada respecto de los dispositivos tipo pívot bilaterales y sus resultados como tratamiento en artralgias de ATM. Palabras clave: Articulación temporomandibular, artralgia, trastornos temporomandibulares. SUMMARYArthralgia of Temporomandibular Joint is a term used in the Diagnostic Criteria for Temporomandibular Disorders (DC/TMD), defined as pain of joint origin that is affected by jaw movement, function, or parafunction, and replication of this pain occurs with provocation testing of the TMJ. In the management of Arthralgia the evidence describes different types of therapies, which can be classified as invasive and non-invasive. The pharmacological and cognitive behavioural therapies, as well as the use of interocclusal appliances are considered among the non-invasive therapies. The interocclusal pivot appliances have been widely used to relieve the TMJ arthralgia.
