Occupational rhinitis: diagnostic criteria, relation to lower airway symptoms and IgE sensitization in bakery workers

Abstract: The prevalence of occupational rhinitis varied between 23% and 50%, depending on the criteria used. The occurrence of nasal symptoms was found to precede the development of lower airway symptoms. Occupational rhinitis, both IgE- and non-IgE-mediated, was associated with asthma symptoms. The most frequent causes of sensitization (20%) were different species of storage mites. Storage mite sensitization was related to occupational rhinitis and work exposure.

“…Several studies have linked exposure to high molecular weight particles in the working environment to development of both IgE-and non-IgE mediated rhinitis and asthma, as in bakers [12,13]. In low molecular weight exposure, the relation is less obvious.…”

“…Immunoglobulin E sensitization to bakery allergens (flour; 139, 591) or enzymes (592) or contaminants (593) seems to be the main cause of bakersÕ asthma and rhinitis but cannot explain nasal or bronchial symptoms in each case (594). Occupational rhinitis, both IgE and non-IgE-mediated, is associated with asthma symptoms (595). Bronchial responsiveness to bakery-derived allergens is strongly dependent on specific skin sensitivity (596).…”
