“…Data from the AHS database (AHS quality collaborative) 74 showed no difference in 30-day morbidity between matched sublay (pooling retromuscular, preperitoneal and intraperitoneal) and onlay repairs of primary ventral and incisional hernias. Two meta-analyses 75,76 , also including both primary ventral and incisional hernias, found that retromuscular mesh placement had the lowest probability of surgical-site infection and recurrence.…”

“…When mesh plug inlay techniques or preformed patches are used, the defect may not be closed as the mesh is fixed to the border of the defect 38,70 . When preperitoneal flat mesh or an onlay mesh is used, the fascial defect is usually closed 48,74 . In the MORPHEUS trial 65 , patients with umbilical hernia were randomized to preperitoneal flat mesh or intraperitoneal patch; the fascia was closed in 86 per cent of patients.…”

“…Ventral hernia repair with onlay mesh placement (over fascia) is technically less challenging than sublay placement (between rectus muscles and rectus sheath). Although placement of mesh over fascia has been associated with higher rate of wound infections based on existing studies, newer case-matched studies showed that onlay mesh placement is a safe alternative in low-risk patients [10]. In patients with large ventral hernias, advanced techniques such as component separation before mesh reinforcement are preferred, as was done in our patient [11].…”

“…Onlay mesh placement is the placement of mesh on the anterior fascia and is sometimes referred to as a premuscular location of mesh [4] . This technique, popularized by Chevrel in 1979, is typically approached in an open fashion with the placement of mesh over the anterior fascia following closure of the fascial defect.…”
