“…Most investigators have recommended internal fixation for posterior malleolar fractures affecting 25% to 33% of the articular surface (13,14). Huber et al (15) demonstrated that indirect reduction from anteriorly to posteriorly restored anatomic joint congruity in only 27% of patients, although it was restored in 83% of patients undergoing a direct posterior surgical approach. Because the posterior malleolar fracture fragments are commonly located on the posterolateral side of the joint, fixation using a posteromedial approach could have disadvantages with regard to the approach and fixation of the posterior malleolar fracture fragment (16).…”

“…It permits unrivalled visualization of the posterior malleolar fracture to enable direct and anatomic reduction with restoration of the articular surface, which is imperative when treating these injuries. Huber et al (17) found that anatomic reduction of the posterior malleolus was achieved in 83% of cases using direct reduction compared with just 27% when indirect reduction was performed. Anatomic reduction will also tension the posterior inferior tibiofibular ligament, which stabilizes the syndesmosis and has been shown to be equivalent to screw fixation of the syndesmosis (3,13).…”
