Open segmental fractures of the tibia treated by external fixation

Abstract: External fixation by the use of Mitković external fixator is one of the methods of choice in the treatment of open segmental tibial fractures, which incorporated with antibiotic therapy provides good biomehanical conditions for segmental fracture healing that enables good stability of the segmental tibial fracture and decreases the risk of deep infections.

“…Although intramedullary nail is the gold standard in the management of tibial diaphyseal fractures, external xation has a clear role in most cases due to the advantage of stabilization with limiting soft tissue dissection, especially for high-energy injuries with poor surrounding soft tissues 3,4,6,8,10,14,17,21 .…”

“…Although open reduction and internal xation (ORIF) which contributes to anatomic reduction has acted as the gold standard for diaphyseal fractures treatment, it is the most invasive form with potential infection risk, especially for high-energy fractures with severe soft tissue damage. Meticulous soft tissue care combined with external xation has given satisfactory clinical results in recent years [3][4][5][6][7] .…”

“…Early parenteral antibiotic administration is associated with the infection incidence decrease and should be applied immediately upon admission. Antibiotic treatment is to be administered during 48 to 76 hours in patients with grade I and II and up to 120 hours in patients with grade III open fractures (9).…”

“…During the injury of the blood vessels, the opening of the lumen can happen (laceration, transection) with resultant bleeding, or obstruction of the flow (thrombosis) and the interruption of circulation. These injuries usually occur in traffic accidents, under the high intensity force, whereas beside lesions of the main blood vessel, there is a great damage of the soft tissues of the lower leg and periosteum removal from the injured bone in the fracture segment 3,8 .…”
