Operation for old sternoclavicular dislocation. Results in 12 cases

Abstract: Twelve patients were operated upon after unsuccessful conservative treatment for complete dislocation of the sternoclavicular joint. Three methods were used; stabilisation using fascial loops, reconstruction with a tendon graft, and resection of the sternal end of the clavicle. The results were good in only four patients, three treated with a tendon graft and one by fascial loops. Another four patients had fair results, but all four treated by resection of the medial end of the clavicle had poor results, with … Show more

“…Various methods have been described, but biomechanical and clinical evidence [6,19,22] suggests that reconstruction of the ligamentous structures of the sternoclavicular joint is imperative for successful fixation. Others have suggested that the costoclavicular ligament represents the vital stabilizing structure [16,17], while others promote the importance of the capsule for limiting migration of the medial end of the clavicle [2,21].…”

“…Eskola et al [22] showed in their work that reconstruction of ligamentous structures with fascial loops [24] seems to give good results; Castropil et al and Martinez et al used semitendinosus and gracilis tendon autografting with good outcome, but with increased morbidity at donor site.…”

“…When also open reduction failed, a partial resection of medial end of clavicle without any further attempt at stabilization is the only way to ride even if with poor functional results [22,23].…”

“…This phenomenon also has been reported after partial or complete claviculectomy by a number of authors. 13,14,16,17,22,32,37 In 1997 Rockwood et al 33 described their surgical technique and the results in 8 patients treated with primary medial clavicle resection for painful SC joints. They developed the rating system used in this study for evaluation of the results of SC joint surgery.…”
