Operative Blood Loss, Blood Transfusion, and 30-Day Mortality in Older Patients After Major Noncardiac Surgery

Abstract: Intraoperative blood transfusion is associated with a lower 30-day postoperative mortality among elderly patients undergoing major noncardiac surgery if there is substantial operative blood loss or low preoperative hematocrit levels (<24%). Transfusion is associated with increased mortality risks for those with preoperative hematocrit levels between 30% and 35.9% and <500 mL of blood loss.

“…This would significantly reduce perioperative complications and the length of hospitalization. So, perioperative alogenous blood transfusion administration that is by itself linked to numerous complications would be thus reduced (26,27). Due to the limitations of our study, further prospective study in larger sample of patients is necessary.…”

“…Perioperative EBL included intraoperative EBL, visual estimation and documentation by the anesthesiologist, and postoperative drain output. The formula by Wu et al [14,15] built from the National Surgical Quality Improvement Program data regarding surgical blood loss (EBL in milliliters = [31.265 * preoperative hematocrit] -[29.83 * postoperative hematocrit] + [269.67 * units of red blood cells transfused]) was used to confirm the accuracy of EBL (p = 0.503; 95% CI, À128.6 to 63.40), Preoperative autologous blood donation was offered to all patients and varied from zero to two units depending on patient preferences. Clinical records from routine followups were available for all patients and checked for incidence of DVT or PE.…”
