Optic nerve sheath diameter sonography during the acute stage of intracerebral hemorrhage: a potential role in monitoring neurocritical patients

Abstract: Background Optic nerve sheath diameter (ONSD) sonography has been proposed as a reliable bedside tool for the detection of increased intracranial pressure (ICP). ONSD reacts almost simultaneously to oscillations in ICP. The aim of this study was to investigate the ONSD dynamics in the acute stage of intracerebral hemorrhage (ICH) and to compare ONSD dynamics to the clinical outcome. Methods We enrolled 35 acute ICH patients and 20 healthy volunteers in this prospective study. At the admission, all patients un… Show more

“…The ONSD values were significantly higher in patients compared to those of controls. This result was already found in a usual way, both in adults ( 8 , 19 , 22 , 23 ) and in children ( 1 , 13 , 24 26 ).…”

“…Furthermore, this small measurement error is insignificant in comparison to the variation in reported ONSD measurement threshold values which vary as much as 1 mm in the literature ( 31 ). Some authors contend that a trend in increasing repeated ONSD measurements or extreme values is more useful in diagnosis to account for these measurement uncertainties ( 32 ). Based on the results of our current study, using one ultrasound unit for repeated measurement may be prudent so as to diminish between-device measurement variability.…”

“…Benefits of this method include ease of application, as the newer method requires careful image acquisition accounting for multiple structures in the orbit. Alternate methods such as trends in measurement over time (32), as well as automated measurement techniques have also been investigated with promising results (37)(38)(39).…”

“…In patients suffering from severe brain damage, ONSD values of about 5.9 to 6.3 mm have been reported [14] with no values lower than 5.8 mm in cases where ICP is >20 mmHg [25]. Therefore, a cut-off value of 5 mm has been suggested [9,26,27]. Despite these numerous studies, there is no consensus regarding a definitive threshold for elevated ICP [21,28,29], which is even true for awake, spontaneously breathing patients with cerebral pathologies.…”

“…In contrast, point of care ultrasonography of optic nerve sheath diameter (ONSD) has become an alternative bedside tool to reproducibly detect intracranial hypertension [9][10][11][12]. The optic nerve is surrounded by all linings of the brain, forming the optic nerve sheath, which connects it to the intracranial space.…”
