Introduction: Preterm labour follows a spontaneous commencement of labour in two-thirds of preterm birth instances. Obstetricians must consider both treatment options for the management of preterm. The study was conducted to compare the tocolytic effects of oral nifedipine tablets and transdermal nitroglycerine patches in preterm labor. Methodology: Cases with Singleton pregnancy, Pregnancy between 28 weeks to 36 completed weeks, and without contraindication for tocolysis were included in the study. Randomly assigned Nifedipine group received oral nifedipine (Tab Depin 10mg) and NTG group received transdermal nitroglycerine patch (Nitroderm 10). Progress was assessed for effect of drug for tocolysis. Results: Both groups were comparable in terms of demographic variable, vital measures, bishop score and cervical dilatation at the time of reporting. Comparison of mean prolongation duration according to gestational age in both the study groups indicate that overall duration of prolongation was better in oral nifedipine. However, the differences were not statistically significant. Headache and hypotension were significantly higher in Nitroglycerine patch compared to oral nifedipine group. The treatment discontinuation was significantly higher in Nitroglycerine patch compared to oral Nifedipine. Conclusion: Tocolytic effect of both, oral nifedipine and Nitroglycerine patch was statistically almost similar. However, considering the feasibility of application, availability, and cost of the drug, we recommend to use oral nifedipine for tocolysis over Nitroglycerine patch.

Backgrounds A significant contributor to newborn morbidity and mortality is preterm birth. Several techniques have been employed to identify patients at risk of premature labour. However, these predictors are not always effective because of their multifactorial aetiology. Preterm labour can be suppressed largely through tocolysis. This study compared the effectiveness and safety of transdermal nitroglycerine and oral nifedipine in preventing premature labour. Methods This study was done at Acharya Vinoba Bhave Rural Hospital, Sawangi, Wardha, Maharashtra, from December 2020 to November 2022, on 130 women presenting with preterm labour pains between 28 and 37 weeks of gestational age. All the women selected were randomized into two equal groups by using the envelope method. Sixty-five women were given a nitroglycerine patch (Group A), and the rest (65 women) were given an oral nifedipine tablet (group B). The variables studied were mean days of prolongation of pregnancy, treatment outcome, steroid coverage, along with feto-maternal outcomes among both groups. Results The percentage of women whose pregnancy was prolonged for at least 48 hours in the nitroglycerine group was 75.3%, and in the nifedipine group it was 93.8%. Failure to achieve tocolysis, defined as delivery within 48 hours, was seen significantly more in the nitroglycerine group (24.6%) than in the nifedipine group (6.1%). The overall foetal outcomes were comparable in both groups. Conclusion Oral nifedipine was found to be superior to transdermal nifedipine patches in terms of efficacy and safety in the management of preterm labour, with a better side effect profile.

Background: Tocolytic agents are used to reduce preterm deliveries. Very few studies documenting the comparison of tocolytic agents viz. nifedipine, nitroglycerin dermal patches and isoxsuprine. Other drugs are not used due to their adverse effects. Objective was to study and compare the safety efficacy of nifedipine, nitroglycerin dermal patches and isoxsuprine as tocolytic agents in suppression of preterm labour 1 year study.Methods: This was a prospective case control study was conducted for a period of 1 year. Total 90 cases selected to study were randomly distribute in to three treatment groups viz. A, B, and C nifedipine, nitroglycerin and isoxsuprine respectively. Subjects in all three groups were evaluated for maternal pulse rate, palpitation uterine contractions and fetal heart rate in order to assess efficacy of each drug under investigation.Results: There was no statistically significant difference in age of woman’s in three different groups. Among (100%) subjects, majority of the cases i.e. (27.8%) primi gravida followed by multi (72.2%). Side effects of nifedipine was less when compared to nitroglycerine dermal patch and isoxsuprine i/v/o of headache (8.9%), nausea (1.1%), vomiting (1.1%), tachycardia (3.3%), palpitation (3.3%), hypotension (1.1%). side effects were statistically significant different between the treatment groups. There was no statistically significant difference with respect to APGAR score at 1 minute and 5 minutes.Conclusions: Oral nifedipine was found to be superior and efficacious as tocolytic agent as compared to transdermal nitroglycerin and intravenous isoxsuprine.
