Oral Pre-Exposure Prophylaxis (PrEP) for Prevention of HIV in Serodiscordant Heterosexual Couples in the United States: Opportunities and Challenges

Abstract: Oral HIV pre-exposure prophylaxis (PrEP) is a promising new biomedical prevention approach in which HIV-negative individuals are provided with daily oral antiretroviral medication for the primary prevention of HIV-1. Several clinical trials have demonstrated efficacy of oral PrEP for HIV prevention among groups at high risk for HIV, with adherence closely associated with level of risk reduction. In the United States (US), three groups have been prioritized for initial implementation of PrEP-injection drug user… Show more

“…Further, our study focused primarily on challenges relating to awareness of PrEP. Awareness, however, is only one barrier to using PrEP; research among other populations at-risk for HIV has identified multiple factors of important consideration in regards to PrEP use, including adherence 47 and comprehensive health care, 48 which will need to addressed in order to make this strategy work.…”

“…11,30 These guidelines also provide clinicians with the information needed to counsel patients about many of the concerns that patients themselves may have about PrEP, including short and long term health effects, the use of PrEP as part of a combination prevention package, and cost. 11,[33][34][35] Including additional topics, such as intermittent use of PrEP and use of PrEP in youth, in future iterations of the guidance may help meet the needs of clinicians who are prescribing PrEP.…”
