Oral pyridoxine can substitute for intravenous pyridoxine in managing patients with severe poisoning with isoniazid and rifampicin fixed dose combination tablets: a case report

Abstract: BackgroundFixed drug combination of isoniazid and rifampicin is a rare cause of poisoning even in endemic countries for tuberculosis infection. Severe poisoning can cause severe morbidity and mortality if not treated promptly. Though intravenous pyridoxine is the preferred antidote for severe standard isoniazid poisoning it is not freely available even in best of care centers. We describe a case of severe poisoning with fixed drug combination of isoniazid and rifampicin successfully managed with oral pyridoxin… Show more

“…With the increasing use of anti-TNF-α biologics, a new group of high-risk patients for tuberculosis (TB) has developed. This has led to the need for increased awareness and effort to screen latent tuberculosis infection (LTBI) with clinical, radiographic, and laboratory measures [4,5].…”

“…Although injectable pyridoxine is easily manufactured, supplies may be difficult to obtain in resource poor settings and a single isoniazid poisoning can exhaust a hospital's stores of pyridoxine 64–67 . Recent data show only 30% of Italian emergency departments stocked pyridoxine, 59 and there are few manufacturers of injectable pyridoxine, which increases risk of supply chain interruptions.…”

“…In 2018 and 2019, there were shortages of injectable pyridoxine due to manufacturing problems 68,69 . Oral pyridoxine, crushed and given via nasogastric tube, has also been used successfully for treating isoniazid poisoning, 42,58,64,70,71 although not preferred as absorption may be impaired in critically ill patients 72 . If pyridoxine is unavailable, GABA A receptor agonists are used, but their effectiveness may be limited.…”
